How should we organize our selection or training procedures? In what way can a flight crew mediate problems? How are we to understand reported errors? Mechanisms in the Chain of Safety presents recent findings in aviation psychology, bringing fresh insights to such questions. Aviation psychologists study personnel selection and training; they evaluate the management of flight operations, and ultimately they analyse the things that went wrong. The strong interrelation between these components allows us to talk about a chain of safety. This volume appraises this chain of safety by considering the mechanisms that determine its effectiveness - input mechanisms, coping mechanisms and control mechanisms. Each contribution discusses a component of the chain while the book as a whole emphasizes and illustrates that understanding the connections between these parts is essential for the future. By addressing these issues the book leads to further considerations such as how mistakes are linked to training and how coping mechanisms should help us to understand errors and accidents. Previous studies conducted within the aviation industry have examined a multitude of crucial aspects such as policy, airline service quality, and revenue management. An extensive body of literature has also recognised the importance of decision-making in aviation, with the focus predominantly on pilots and air traffic controllers. Understanding Decision-Making Processes in Airline Operations Control focuses instead on an area largely overlooked: an airline's Operations Control Centre (OCC). This serves as the nerve centre of the airline and is responsible for decision-making with respect to operational control of an airline's daily schedules. The environment within an OCC is extremely intense and a key role of controllers is to make decisions that facilitate the airline's recovery from frequent, highly complex, and often multiple disruptions. As such, decision-making in this domain is critical to minimise the operational, commercial and financial impact resulting from disruptions. The book examines many aspects of individual decision-making in airline operations, and addresses the deficiencies found by presenting to the reader an examination of the relationships among situation awareness, information completeness, experience, expertise, decision considerations and decision alternatives in OCCs. The text utilises a multiple case study approach and proposes a number of relevant and important implications for OCC management. What does the collapse of sub-prime lending have in common with a broken jackscrew in an airliner's tailplane? Or the oil spill disaster in the Gulf of Mexico with the burn-up of Space Shuttle Columbia? These were systems that drifted into failure. While pursuing success in a dynamic, complex environment with limited resources and multiple goal conflicts, a succession of small, everyday decisions eventually produced breakdowns on a massive scale. We have trouble grasping the complexity and normality that gives rise to such large events. We hunt for broken parts, fixable properties, people we can hold accountable. Our analyses of complex system breakdowns remain depressingly linear, depressingly componential - imprisoned in the space of ideas once defined by Newton and Descartes. The growth of complexity in society has outpaced our understanding of how complex systems work and fail. Our technologies have gotten ahead of our theories. We are able to build things - deep-sea oil rigs, jackscrews, collateralized debt obligations - whose properties we understand in isolation. But in competitive, regulated societies, their connections proliferate, their interactions and interdependencies multiply, their complexities mushroom. This book explores complexity theory and systems thinking to understand better how complex systems drift into failure. It studies sensitive dependence on initial conditions, unruly technology, tipping points, diversity - and finds that failure emerges opportunistically, non-randomly, from the very webs of relationships that breed success and that are supposed to protect organizations from disaster. It develops a vocabulary that allows us to harness complexity and find new ways of managing drift.
Over the past fifteen years or so, there has been a widespread and increasing fascination with the theme of mobility across the social sciences and humanities. Of course, geographers have always had an interest in mobility, but as yet they have not viewed this in the same 'mobility turn' as in other disciplines where it has been used to critique the standard approaches to the subjects. This text brings together leading academics to provide a revitalised 'geography of mobilities' informed by this wider 'mobility turn'. It makes connections between the seemingly disparate sub-disciplinary worlds of migration, transport and tourism, suggesting that each has much to learn from each other through the ontological and epistemological concern for mobility.
Resilience engineering has since 2004 attracted widespread interest from industry as well as academia. Practitioners from various fields, such as aviation and air traffic management, patient safety, off-shore exploration and production, have quickly realised the potential of resilience engineering and have became early adopters. The continued development of resilience engineering has focused on four abilities that are essential for resilience. These are the ability a) to respond to what happens, b) to monitor critical developments, c) to anticipate future threats and opportunities, and d) to learn from past experience - successes as well as failures. Working with the four abilities provides a structured way of analysing problems and issues, as well as of proposing practical solutions (concepts, tools, and methods). This book is divided into four main sections which describe issues relating to each of the four abilities. The chapters in each section emphasise practical ways of engineering resilience and feature case studies and real applications. The text is written to be easily accessible for readers who are more interested in solutions than in research, but will also be of interest to the latter group.

New Ergonomics Perspective represents a selection of the papers presented at the 10th Pan-Pacifi c Conference on Ergonomics (PPCOE), held in Tokyo, Japan, August 25-28, 2014. The first Pan-Pacific Conference on Occupational Ergonomics was held in 1990 at the University of Occupational and Environmental Health, Japan. The main theme of the PPCOE 1990 was "Searching for Solutions to Occupational Problems." The successful meetings have taken place in 1990 (Kitakyushu, Japan), 1992 (Wuhan, China), 1994 (Seoul, Korea), 1996 (Taipei, Taiwan), 1998 (Kitakyushu, Japan), 2001 (Beijing, China), 2004 (Cairns, Australia), 2007 (Bangkok, Thailand) and 2010 (Kaohsiung, Taiwan). After two decades of collective endeavors, the conferences have established globally recognized excellence in scientific and professional contributions. Now, PPCOE activities are entering a new stage after a quarter century of history. Fortunately, participants of the PPCOE Final in Tokyo are eager to take a first step on the road to its rebirth: the establishment of a new, mature PPCOE. PPCOE pursues ergonomics as a practical science, covering a broad spectrum of up-to-date and comprehensive ergonomics issues. This book was organized into six thematic chapters, including Aging and Occupational Safety, Workplace Ergonomics, Occupational Ergonomics, Healthcare and Special Populations, Human Computer Interaction and Current Issues and Ergonomics Approach.
This special issue contains essays regarding the CHI '95 conference, which featured a panel titled, Discount or Disservice? Discount Usability Analysis: Evaluation at a Bargain Price or Simply Damaged Merchandise? Wayne Gray, who organized the panel, presented a controversial critique of studies that had evaluated various usability evaluation methods (UEMs). The level of interest in this discussion led Gray to propose a review article that dealt with the issues in a more systematic fashion. The resulting essay, written by Gray and his collaborator Marilyn Salzman, conducted an in-depth review of a series of influential studies that used experimental methods to compare a variety of UEMs. Gray and Salzman's analysis was framed using Cook and Campbell's (1979) well-known discussion of various forms of validity. They used this to evaluate numerous details of these comparative studies, and they concluded that the studies fell short on the criteria by which good experimental studies are designed and interpreted.
The last few decades have been among the most dynamic within recent British cultural history. Artists across all genres and media have developed and re-fashioned their practice against a radically changing social and cultural landscape ? both national and global. This book takes a fresh look at some of the themes, ideas and directions which have informed British art since the later 1980s through to the first decade of the new millennium. In addition to discussing some iconic images and examples, it also looks more broadly at the contexts in which a new ?post-conceptual? generation of artists, those typically born since the late 1950s and 1960s have approached and developed aspects of their professional practice. Human error is cited over and over as a cause of incidents and accidents. The result is a widespread perception of a 'human error problem', and solutions are thought to lie in changing the people or their role in the system. For example, we should reduce the human role with more automation, or regiment human behavior by stricter monitoring, rules or procedures. But in practice, things have proved not to be this simple. The label 'human error' is prejudicial and hides much more than it reveals about how a system functions or malfunctions. This book takes you behind the human error label. Divided into five parts, it begins by summarising the most significant research results. Part 2 explores how systems thinking has radically changed our understanding of how accidents occur. Part 3 explains the role of cognitive system factors - bringing knowledge to bear, changing mindset as situations and priorities change, and managing goal conflicts - in operating safely at the sharp end of systems. Part 4 studies how the clumsy use of computer technology can increase the potential for erroneous actions and assessments in many different fields of practice. And Part 5 tells how the hindsight bias always enters into attributions of error, so that what we label human error actually is the result of a social and psychological judgment process by stakeholders in the system in question to focus on only a facet of a set of interacting contributors. If you think you have a human error problem, recognize that the label itself is no explanation and no guide to countermeasures. The potential for constructive change, for progress on safety, lies behind the human error label.
Soldier-robot teams will be an important component of future battle spaces, creating a complex but potentially more survivable and effective combat force. The complexity of the battlefield of the future presents its own problems. The variety of robotic systems and the almost infinite number of possible military missions create a dilemma for researchers who wish to predict human-robot interactions (HRI) performance in future environments. Human-Robot Interactions in Future Military Operations provides an opportunity for scientists investigating military issues related to HRI to present their results cohesively within a single volume. The issues range from operators interacting with small ground robots and aerial vehicles to supervising large, near-autonomous vehicles capable of intelligent battlefield behaviors. The ability of the human to 'team' with intelligent unmanned systems in such environments is the focus of the volume. As such, chapters are written by recognized leaders within their disciplines and they discuss their research in the context of a broad-based approach. Therefore the book allows researchers from differing disciplines to be brought up to date on both theoretical and methodological issues surrounding human-robot interaction in military environments. The overall objective of this volume is to illuminate the challenges and potential solutions for military HRI through discussion of the many approaches that have been utilized in order to converge on a better understanding of this relatively complex concept. It should be noted that many of these issues will generalize to civilian applications as robotic technology matures. An important outcome is the focus on developing general human-robot teaming principles and guidelines to help both the human factors design and training community develop a better understanding of this nascent but revolutionary technology. Much of the research within the book is based on the Human Research and Engineering Directorate (HRED), U.S. Army Research Laboratory (ARL) 5-year Army Technology Objective (ATO) research program. The program addressed HRI and teaming for both aerial and ground robotic assets in conjunction with the U.S. Army Tank and Automotive Research and Development Center (TARDEC) and the Aviation and Missile Development Center (AMRDEC) The purpose of the program was to understand HRI issues in order to develop and evaluate technologies to improve HRI battlefield performance for Future Combat Systems (FCS). The work within this volume goes beyond the research results to encapsulate the ATO's findings and discuss them in a broader context in order to understand both their military and civilian implications. For this reason, scientists conducting related research have contributed additional chapters to widen the scope of the original research boundaries.
