Key Feature of this Manual Include * A brief discussion of the basic pharmacology of each neurocritical drug, with an emphasis on how to select and use these drugs in multiple clinical contexts. * 150 drugs accompanied by a diagram for quick comprehension and drug administration guides. * Unique blending of expertise of neurointensivist with a critical care pharmacist to provide a vital resource for both specialities * References for further reading that are oriented toward utility in clinical practice.
This is a comprehensive but concise reference that documents the nature and importance of the injury problem in the United States. For each of more than sixty causes of injury, data are presented by age, race, sex, geographic area, urban/rural residence, and per capita income. This second edition includes new chapters on injuries related to sports, work, aviation, and large trucks. Also new are many analyses subdivided by four racial groups as well as age and sex, made possible by the use of mortality data from a seven year period. The updated analyses of time trends throughout the book document major reductions in death rates over the past decade. As a statistical compilation, the book offers users a quick reference to valuable detail, much of which would otherwise be inaccessible. It also discusses reasons for many of the extreme differences among groups of people in injury death rates and describes promising avenues to prevention. This accessible, readable reference will be valuable to public health personnel, physicians, epidemiologists, safety planners and policy makers.

This 14th volume of Advanaes in Neurosurgery includes the papers presented at the 36th Annual Meeting of the German Society of Neurosurgery in Berlin, May 12-15, 1985. I would like to take this opportunity to thank the members of the program committee of the Society, Priv.-Doz. Dr. Klinger, Professors Brock, Dietz, Frowein, Lausberg, Wlillenweber, and Dr. Reuter for their assistance in selecting the contributions and in organizing the scientific program. The first main topic of the meeting was Spinal Cord Tumors. Introductory lectures dealing with basic anatomic knowledge, neuropathological as pects, and neurologic problems were followed by reports on examinations using conventional neuroradiology, scintiscanning, computer tomography as well as the most recent method in the diagnosis of spinal tumors, the magnetic resonance tomography. Also presented were the results of a multicentered study on spinal tumors, ascertained in cooperation with 43 German and Austrian neurosurgical clinics. The participants reported in great detail on the statistical data they recorded from 3056 cases and on the scientific findings obtained from this study. The session concluded with lectures on the possibilities for surgical treatment of spinal tumors and on oncologic and radiotherapeutic mea sures. E: x: perimental Neurosurgery was the second main topic. Leading authorities in the field presented interesting papers on topics such as the therapy of vasculogenetic brain edema, the determination of neurotransmitters in brain tumors, results of cerebral blood flow measurement, and im proved operative techniques using laser radiation."
The significant strides made during the past decade in neonatal medicine are based largely upon the provision of intensive perinatal care to the parturient, her fetus, and the newborn. In addition to technologic advances in the bioelec- tronic monitoring of the fetus and newborn, the introduction of noninvasive methods to monitor oxygenation, the pharma- cologic manipulation ofthe ductus arteriosus and pulmonary vasculature, and the use of ultrasonographic diagnosis of congenital heart defects and other lesions, intensive efforts by neonatal specialists throughout the world have also contri- buted to the decline in neonatal and infant mortality. Professors Wille and Obladen have provided an authoritative manual outlining intensive care of newborns and the ways in which the science of neonatal medicine, the technology of the 1980 s, and the art of application have resulted in the practice of neonatal intensive care at the Kinderklinik of the University of Heidelberg. The English translation resulted from the success of the German edition and will serve well the needs of the English-speaking student and practitioner of neonatal intensive care. T. Allen Merrit, M. D.

Inflammation in itself is not to be considered as a disease . . . and in disease, where it can alter the diseased mode of action, it likewise leads to a cure; but where it cannot accomplish that solitary purpose . . . it does mischief - John Hunter, A Treatise on the Blood, ITfIlammation, and Gunshot Woundr (London, 1794)1 As we reached the millennium, we recognized the gap between our scientific knowledge of biologic processes and our more limited clinical capabilities in the care of patients. Our science is strong. Molecular biology is powerful, but our therapy to help patients is weaker and more limited. For this reason, this book focuses on the problems of multiple organ failure (MOF), multiple organ dysfunction syndrome (MODS), and systemic inflammatory response syndrome is, patients who have severe injuries; require major, (SIRS) in high-risk patients, that overwhelming operations; or have serious illnesses requiring intensive care; patients who have diseases elsewhere, in other organs or systems, that limit their capabilities to survive a new insult; and patients who are elderly or at high risk for sepsis or other complications. These are the patients who need our help. They need the advances in science, in molecular biology, immunology, pathophysiology, biochemistry, genetics, high technology, and other areas of maximum support at the bedside. These advances could potentially have the greatest impact on improving patient care.

Simplify, simplify! Henry David Thoreau For writers of technical books, there can be no better piece of advice. Around the time of writing the first edition - about a decade ago - there were very few monographs on this s- ject: today, there are possibly no less than 20. Based on critical inputs, this edition stands thoroughly revamped. New chapters on ventilator waveforms, airway humidification, and aerosol therapy in the ICU now find a place. Novel software-based modes of ventilation have been included. Ventilator-associated pneumonia has been se- rated into a new chapter. Many new diagrams and algorithms have been added. As in the previous edition, considerable energy has been spent in presenting the material in a reader-friendly, conv- sational style. And as before, the book remains firmly rooted in physiology. My thanks are due to Madhu Reddy, Director of Universities Press - formerly a professional associate and now a friend, P. Sudhir, my tireless Pulmonary Function Lab technician who found the time to type the bits and pieces of this manuscript in between patients, A. Sobha for superbly organizing my time, Grant Weston and Cate Rogers at Springer, London, Balasaraswathi Jayakumar at Spi, India for her tremendous support, and to Dr. C. Eshwar Prasad, who, for his words of advice, I should have thanked years ago. vii viii Preface to the Second Edition Above all, I thank my wife and daughters, for understanding.

The practice of intensive care medicine raises multiple legal and ethical issues on a daily basis, making it increasingly difficult to know who to admit and when, at what stage invasive management should be withdrawn, and who, importantly, should decide? These profound dilemmas, already complicated in a setting of scarce resources, mandate an understanding of law and ethics for those working in intensive care medicine. Clinically focused, this book explains the relevance of landmark rulings to aid your day-to-day decision-making. A spectrum of ethical and legal controversies in critical care are addressed to demonstrate how law and ethics affects the care available to patients and vice versa. Discussion of conflict resolution advises the options open to you when agreement on treatment decisions or withdrawal cannot be reached. The literature and variations surrounding Do Not Attempt Resuscitation decisions are outlined to help you navigate this complex area. This edition also provides an up-to-date analysis of issues such as futility and depreciation of liberty. Featuring contributions from leading legal and medical experts, this important reference should be read by every critical care professional.

The end of the second millenium is distinguished for the increasing interest in the field of critical care medicine, not only among physicians and clinical scientists but also on the part of the mass media. This is an interdisciplinary area of medicine drawing upon the specialties of anesthesiology, internal medicine and surgery, and relying upon the essential contributions and support from basic research. Advances in critical care medicine depend on the application of new technologies to the clinic, the full integration of computers and informatics, the continual training of physicians and technicians, and the consideration of ethical issues in the clinical setting. Within this complex panorama of complementary approaches and viewpoints, it is apparent that critical care medicine is one of the best examples of evidence-based medicine. |
