1 Allgemeines Wahrend der friihen 70er Jahre sind die modemen Schnittbildverfahren Sono- graphie und Computertomographie entwickelt worden, welche die nichtinvasi- ve Diagnostik im Bereich der parenchymatOsen Organe betrachtlich verbessert haben und die Beurteilung krankhafter Vedinderungen in Korperregionen er- lauben, die zuvor nur mit Schwierigkeiten, invasiv, indirekt oder unzureichend untersucht werden konnten. Diese Schnittbildverfahren haben heute einen hohen Stand der Perfektion erreicht und stehen nicht nur in allen groBen, sondem oft auch in kleineren Zentren und sogar in mancher Praxis zur Verfiigung. Damit kommen ihre Vorteile den Kranken voll zugute. In jiingster Zeit gesellt sich zu ihnen noch eine weitere Schnittbildmethode, die Kemspintomographie (MR!, "nuclear magnetic resonance"), die ohne Rontgenstrahlen arbeitet und Tomographien in verschiedenen Ebenen anzufertigen erlaubt. Der diagnostisch tatige Arzt wird heute in die Lage versetzt, bei manchen Kranken mit Hilfe der modemen Schnittbildtechnik rasch die endgtiltige Dia- gnose zu finden. Gelingt dies nicht auf Anhieb, so helfen ihm diese Verfahren, die Wahl fUr jene Folgeuntersuchung zu treffen, die am ehesten bei der Fahn- dung nach dem zugrunde liegenden KrankheitsprozeB weiterhilft. Die Ultrasehall-Methodik setzt sieh mehr und mehr im klini- sehen Alltag als diagnostisehe Eingangsuntersuehung bei al- len Fragen naeh abdominellen Massenlasionen oder Steinen dureh - stark begiinstigt einerseits dureh die vollig fehlende ionisierende Strahlung und andererseits dureh die zuneh- mend aufwendiger werdenden GroBgerate-Methoden, wie Computertomographie, digitale Subtraktionsangiographie, Szintigraphie und Nuklear-Magnetie-Resonanee-Verfahren, die zudem allesamt noeh Wartezeiten erfordem. Immer wieder fasziniert bei der Ultrasehall-Methodik die groBe diagnostisehe Zuverlassigkeit und Relevanz des Ver- fahrens in der Hand des Geiibten. Der erfahrene Arzt steht hier ganz im Mittelpunkt der diagnostisehen Aufklarung. Frau Dachs, 75 Jahre, hat das Aussehen eines jungen Madchens. Sie verliert jedoch Gewicht und klagt uber epigastrische Schmerzen. Sie hat eine ganze Reihe konventioneller radiologischer Verfahren uber sich ergehen lassen mus- sen. Dies ist fur die Roentgenfilmhersteller eine gute Nachricht, hat zur Diagnose jedoch nichts beigetragen. Zuletzt wird sie zu einer Ultraschalluntersuchung uberwiesen. Sehen Sie sich zunachst die Schnittbilder 1.Ia und 1.Ib (transversal) und 1.Id (sagittal) an.

Die Bilddarstellung der Prostata mittels transrektaler, transver- saler Sonographie und die sich daraus ergebende klinische Re- levanz wurde erstmals in Japan erarbeitet. Wahrend TAKAHASHI und OUCHI 1964 uber die prinzipielle Methodik berichteten, . konnte WATANABE 1967 erste klinische Ergebnisse zeigen. Von ihm gingen auch in den folgenden Jahren wesentliche Impulse in der Weiterentwicklung dieser Untersuchungsmethode aus, die dann in Europa und Amerika u. a. uberwiegend von HOLM, DENIS, SCHROEDER, FRENTZEL-BEYME sowie von RESNIK und KING aufgegriffen wurden. Zahlreiche Veroeffentlichungen zur Pathologie der Prostata bekundeten das Interesse an der Dia- gnostik dieses Organs. Die Haufigkeit der malignen Erkrankung der Prostata erklart das Bemuhen, die bereits etablierten klini- schen Methoden durch eine zusatzliche Methode zu erganzen. Durch die Einfuhrung der Grauwertskala in der transrektalen Sonographie wurde eine deutliche Verbesserung der Bildqualitat und somit der diagnostischen Aussagen erreicht. Die Beitrage in dem 1981 erschienenen Buch von WATANABE, HOLMES, HOLM und GOLD BERG belegen dies. Das vorliegende Buch entstand in der Absicht, einen zusam- menfassenden UEberblick uber den Stand der transrektalen, transversalen Diagnostik zu geben, die sowohl im Rahmen der Vorsorge als auch des Stagings und des Verlaufs der Prostataer- krankungen inzwischen mehr und mehr Anwendung findet. Die Erfahrungen, die in diesem Buch zusammengefasst wurden, konnten nur im intensiven interdisziplinaren Gesprach zustande kommen.
