The intention of the VIIth International Symposium on the Facial Nerve was to create a platform for an extensive exchange of knowledge and scientific infor mation between clinicians and basic research workers. This aim could only be realized on the basis of a common interest in the facial nerve, which unites the interdisciplinary scientific efforts of otologists, neurosurgeons, facial plastic surgeons, neurologists, neurophysiologists, and neuroanatomists. Therefore, a meeting of this kind remains in its aim to exchange ideas over scientific disciplines which do not come unique together normal conditions. The symposium has been held every four years since 1966. The VIIth sym posium was preceded by symposia in Stockholm (1966), Osaka (1970), ZUrich (1976), Los Angeles (1980), Bordeaux (1984) and Rio de Janeiro (1988), and marked an important milestone in the continuously developing knowledge about the facial nerve, its physiology, disorders, diagnostics and treatment. In contrast to the previous meetings this symposium extended in vitations to both clinicians and basic research workers. More than 350 scientists from 25 different nations met in Cologne, Germany, in June 1992 and their high-level presentations contributed to the overwhelming success of this international meeting. The symposium took place in the vicinity of the old cllthedral of Cologne, itself a vivid symbol of never-ending efforts to create something perfect and lasting."

This volume deals with tumours occurring in the following sites: Nasal cavity and paranasal sinuses Nasopharynx Larynx, hypopharynx and trachea External ear Middle and inner ear The classification of tumours arising from each of the anatomical sites listed above is given separately. However, since the same tu- mour type may occur in more than one site, the definitions and il- lustrations have been grouped together. The classification is based on the histological characteristics of the tumours and is therefore concerned with morphologically identifiable cell types and histo- logical patterns as seen with conventional light microscopy. Al- though many of the histological terms and definitions have histo- genetic implications, this classification is not primarily based on histogenesis. The term "tumour" is used synonymously with neoplasm. The term "tumour-like" is applied to non-neoplastic lesions which clini- cally or morphologically resemble neoplasms; they are included in this classification because of their importance in differential diag- nosis. The terminology adopted for individual tumours is based on their general acceptance and world-wide usage. Synonyms are in- cluded only if they have been widely used in the literature or if they are considered helpful in understanding the lesions. In such cases the preferred terms are given first followed by the synonyms within parentheses.
This second edition is more extensive and detailed than the previous one - now a reference of two decades standing. New technical methods have increased our understanding of many tumours, and the earlier group of monomorphic adenomas has been separated for easier identification, while clearly defined tumours - even if uncommon - have their own category. With reference to their differences in prognosis and treatment, the various types of carcinomas are now listed separately, and new entities have been adopted. The inclusion of the TNM classification of salivary gland tumours together with the more detailed description of the tumour-like lesions will contribute to an even better understanding.
Fibrin plays a prominent role in wound healing. It has a hemostatic effect, induces cellular response to wound damage,' and, by forming strands to build a matrix, assists in neovascularization and fibroblast proliferation. The concept of using clotting substances from human blood for wound manage- ment and to achieve hemostasis in bleeding parenchymatous organs can be traced to 1909, when Bergel [1] reported on the hemostatic effect of fibrin powder. In 1915, Grey [3] employed fibrin to control bleeding in neurosurgical operations of the brain. A year later, Harvey [4] used fibrin patches to stop bleeding from parenchy- matous organs in general surgery. It took more than two decades for this ingenious idea to be rediscovered. In 1940, Young and Medawar [8] reported on experimental nerve anastomosis by sealing. Similarly, Tarlov and Benjamin [7] reunited nerves with plasma clots in 1943. Tarlov improved the results obtained with clot anastomosing of nerves by avoiding tension at the nerve stumps. In 1944, Cronkite et al. [2] reported on an initial series of eight cases in which fibrinogen and thrombin had been used successfully for anchoring skin grafts.

Incidence of invasive fungal sinusitis has been increasing over the years. The understanding of its pathophysiology has improved with newer serological tests and diagnostic methods helping in earlier diagnosis and reducing patient morbidity. It was believed earlier that invasive fungal sinusitis is seen only in immune compromised patients but clinical reports suggest otherwise. More anti-fungal drugs are being added to improve incidence of patient survival. This book aims to compile all practical information about invasive fungal sinusitis into a single volume.
