This is a book written for those of us who evaluate the many quirks and behaviors encountered in the lives of developing children. The number of important pathologic conditions that befall this age group is staggering. However, equally staggering is the number of episodic and often benign or developmentally programmed paroxysmal events encountered in this population. The nature of these episodic events in many circumstances leads to their erroneous identification as epileptic in nature. It is my hope that this book will serve as a compendium of those episodically manifest entities often uniquely observed in childhood. The book is organized with some foundations in terminology and an in depth analysis of epidemiology of many of these disorders. As best possible, epidemiologic data are compiled with incidence and prevalence figures provided for comparisons. Discussion on the deductive approach to their clinical assessment is provided along with emphasis on the diagnostic clues contained within the descriptive and observational information provided during clinical evaluation. A categorization of the differential features and ages of presentation are provided as a means to help direct logical diagnostic considerations. Psychiatric disorders in adolescents are an important social problem which is relevant to almost all healthcare professionals. According to the results of The National Comorbidity Survey-Adolescent Supplement (NCS-A), the lifetime prevalence of anxiety, behavior, mood, and substance use disorders among adolescents was 31.9%, 19.1%, 14.3%, and 11.4%, respectively. Approximately 40% of participants in this survey with one class of disorder also met criteria for another class of lifetime disorder. Comorbidity is increasingly recognized as a key feature of mental disorders among adolescents. Female adolescents are more likely than males to have mood and anxiety disorders, but less likely to have behavioral and substance use disorders. Regretfully, medical professionals are not sufficiently trained about adolescent psychiatric disorders. For example, primary care providers correctly identify less than a fourth of youth with a depressive or anxiety disorder. Also, many clinicians underestimate the importance of the problem of adolescent psychiatric illnesses and suicidal behavior. Lack of skilled medical providers impedes the delivery of needed services to adolescents with mental health issues. This coupled with a lag in the ability of primary health care services to incorporate psychiatric interventions, and a failure of public health initiatives to pay attention to adolescent mental health problems has led to continuing gaps in care over decades despite the public pronouncements of needs. Over the past two decades, the assessment of Attention-Deficit/Hyperactivity Disorder (AD/HD) has evolved into a sophisticated balance of science and clinical judgement essential for arriving at reliable and valid diagnostic de- sions. Because of the precarious mix of clinical and empirical skill needed to evaluate children with this disorder, diagnostic practice in this area has been found wanting by many critics.
