The nonsurgical management of gallstone disease has drawn widespread clinical interest during the last decade as ultrasound surveys have indicated that cholelithiasis is predom inantly an asymptomatic condition and much more prevalent than previously thought. This book presents an overview of the pathophysiologic and pathobiochemical principles of gallstone formation and the consequences for clinical therapeutic regimens. New information concerning the balance between vesicular and micellar transport of choles terol, early cholesterol nucleation, and the influence of inhibiting and promoting com pounds for cholesterol nucleation as well as information concerning the effects of changes in gallbladder motility and gallbladder mucosal function is reviewed. In order to make further progress in developing treatments which facilitate gallstone dissolution and in preventing disease, it is necessary to integrate this new data into our thinking. Methods of treatment such as systemic litholysis of cholesterol gallbladder stones with bile acid preparations and mechanical fragmentation of stones either by extracorpo ral shock waves or intracorporal laser systems are carefully described and separate discussions of direct contact litholysis of cholesterol stones with ether preparations and the local litholytic treatment of calcified pigment stones are included. New therapeutic applications of HMG-CoA-reductase inhibitors and nonsteroidal anti-inflammatory drugs are also critically reviewed. Finally, for the interested reader an evaluation of prophylactic treatment against stone recurrence after successful conservative treatment and an appraisal of alternative management strategies supplement the information on the conservative treatment of gallstones.
Emerging Electromagnetic Medicine presents the latest research findings in the field of electromagnetic radiation. The book discusses the research of current active researchers in terms of synthesizing and sharing their ideas on the use of electromagnetic fields in diagnostic and therapeutic medicine, now and in the future, including the benefits and risks involved. The book consists of three parts prefaced by a brief historic perspective. Section I describes the theoretical concerns and actual mechanisms involved; Section II covers current preclinical studies, performed in vivo and in vitro, concerning the biological action of the electromagnetic radiation; and Section III reports on actual clinical applications of electromagnetic therapy and also the current machinery used to do so.
Interventional Radiology has as its main goal the performance of surgical techniques using a percutaneous approach to simplify patient care. Percutaneous cholecystostomy now has many advocates; still, it is practised in comparatively few centers. Over many years it was used as a last resort at failed transhepatic cholangiography to provide images of the bile ducts in biliary obstruction. Transhepatic cholecystostomy is reputed to be safer than transperito- neal puncture, since bile leaks do not enter the peritoneum. The advo- cates of percutaneous cholecystolithotomy, almost without exception, fa- vour subcostal cholecystostomy and puncture of the fundus of the gall- bladder. There is no evidence of bile peritonitis after successfully making a track to the gallbladder 18 F in diameter or larger for stone removal. After 1-7 days a postlithotomy drain is removed from the gallbladder and the patient is allowed home. Transhepatic cholecystostomy for gallstone lysis, in contrast, requires only a 5-F track to the hepatic surface of the gallbladder. Loss of the gallbladder is not as great a fear with this technique as it is during dilata- tion of a subcostal track for cholecystolithotomy. During the latter pro- cedure this may result in laparotomy to avoid bile peritonitis, while in the former, if the gallbladder is still visualised, the procedure may be re- commenced immediately. Catheter dislodgement is a fear when prolon- ged catheterisation is considered.
Why and Wherefore? Welcome to our little introductory book It appears in response to be ginning students (i.e., especially general radiology residents) who have sought guidance with the methodology of evaluating positional rela tionships of the feet from radiographs. Since my stepwise "how to" technique has been well received both in the Show-Me state (at the University of Missouri.Columbia) and here in Ohio, it is offered to you as well. To simulate my conventional teaching method, the informal text has an interactive flavor, which I hope makes it useful for you. Please don't be offended by a touch of simplified walking.through of material here and there. After all, this is a "primer," not a dignified postgraduate treatise. Comments from readers would of course be appreciated by the author. The majority of illustrations are the result of the collaboration with my dear colleague Tamar, in which she artistically interpreted repre sentative radiographs for you to my specifications. The guidelines I gave her included line drawing outlines of the most pertinent bones, to which straight lines were added as appropriate. As a result certain bones, for example the fibula and many phalanges, are omitted if they are unlikely to enhance the impact of the illustrations. Often, too, the cuneiforms and cuboid remain undifferentiated as one bony element, or only a few of them are drawn, so that attention remains with the key bones involved."
The 17th volume of the "Advances in Neurosurgery" contains a selection of the scientific reports of the 39th annual meeting of the German Society for Neurosurgery, which was held in Cologne from May 8-11, 1988. The first section deals in particular with the "long-term results of severe head " "injuries," as well as with problems of the acute traumatic hematomas and brain edema. The second section covers "microsurgical experiences." Attention is focused on the anatomy and operative technique for lesions in and around the "jugular foramen" and the craniospinal transition. The third section describes the special new results of "brain " "death determination." Beyond this, numerous contributions on clinical and research results were presented in a poster exhibition which was systematically studied in order to give younger neurosurgeons the opportunity for extensive discussion. The volume contains new information which will bring specialists up-to-date on the latest clinical and research developments in the field.
The biological action of radiation undoubtedly constitutes an issue of actual con cern, particularly after incidences like those in Harrisburg or Chernobyl. These considerations, however, were not the reason for writing this book although it is hoped that it will also be helpful in this respect. The interaction of radiation with biological systems is such an interesting research objective that to my mind no special justification is needed to pursue these problems. The combination of physics, chemistry and biology presents on one hand a fascinating challenge to the student, on the other, it may lead to insights which are not possible if the dif ferent subjects remain clearly separated. Special problems of radiation biology have quite often led to new approaches in physics (or vice versa), a recent example is "microdosimetry" (chapter 4). Biological radiation a9tion comprises all levels of biological organization. It starts with the absorption in essential atoms and molecules and ends with the development of cancer and genetic hazards to future generations. The structure of the book reflects this. Beginning with physical and chemical fundamentals, it then turns to a description of chemical and subcellular systems. Cellular effects form a large part since they are the basis for understanding all further responses. Reactions of the whole organism, concentrating on mammals and especially humans, are subsequently treated. The book concludes with a short discussion of problems in radiation protection and the application of radiation in medical therapy. These last points are necessarily short and somewhat superficial."
During the last few decades, there has been a tremendous improvement in the treatment of cancer. There is evidence that this trend is continuing, based on the achievements re sulting from the combined efforts of clinicians and basic re search workers. This book is an example of such interaction and collaboration. It was prepared by authors representing both areas of work. Most of the work reported in this book is not merely theoretical, but has been experimentally successfully tested and sometimes applied clinically. This work has, however, not yet been generalized and practiced on a wide scale. Some of the results reported here relate to new aspects and open new horizons for future progress. This book will be of great value for both clinicians and basic research workers. Today, all malignant neoplasms are managed by one or more of these modalities, with varying success rates depending on the type of tumor, its degree of spread, and the knowl edge and skill with which the treatment plan has been designed and executed. In the United States of America and in Europe, approximate ly half of all cancer is presently curable, but this has been the case for approximately 20 years."
In every area of human endeavor, technology has opened the door for new advancements to occur. Much of the progress in medicine over the last few years is due, in large part, to new technological tools made available to clinicians and researchers. Laser is an expanding technological discipline in medicine that will ultimately contribute to a broad and rapid expansion of both diagnostic and treatment procedures. Laser is to light what music is to noise. Those physicians who wish to be most successful in the application of this technology, to the benefit of their patient, will learn of the subtle interactions of light with tissue. No technology is good or bad in itself. It is only in the choices we make, in when and how to apply that technology, that it gains its moral value. The use of lasers in medicine has some very definite advantages in the surgical and medical treatment of a variety of disorders. At the same time we must all be careful to not perpetrate the myth of lasers in medicine. Vastly overstated claims of the value of 'laser surgery' have been held out to the general public, resulting in health care being sought on the basis of laser availability.
Technical advances in the field of mammography and better interpretation of the photographs have led recently to a more precise and reliable means of diagnosis, especially for findings which are difficult to analyse. With the help of numerous pathological and control cases the authors explain both the step-by-step procedure in diagnosis and the advantages of mammography as the choice of examination in particular patients. This book guides the reader to an exact interpretation of mammograms. The book is directed at radiologists, gynaecologists and general practitioners. Interdisciplinary collaboration and diagnosis in the preclinical stage are especially important for furthering the prevention of breast cancer.
These exercises are meant for students and practitioners who wish to familiarize themselves with the normal and pathologieal computerized tomographie radioanatomy of the abdomen. The iconography is suffieiently characteristic to be read without the help of clinical or biological data. It comprises both normal and pathologie findings. Analysis of scans is comprised of two steps. The first part consists of the detailed study of normal scans, whieh serve as a reference. For this, eight main slice levels have been considered necessary and sufficient: neces sary since a certain number of slices are indispensable for the exploration of the abdomen; sufficient because a larger number of slices would risk rendering memorization difficult. The second part involves a study of the pathologie findings, organ by organ. Acknowledgements. This numbered radio graph of the abdomen is an analogous representation of the information and allows the location of the eight selected slice levels; these are represented by horizontallines. The slices are 10 mm thick and are taken at intervals of 2.5 cm.
This small but information-packed book is the first to focus exclusively on iatrogenic vascular injuries. It is a timely first, for the scope and magnitude of this subject have reached almost epidemic proportions recently, as a result of exponential increases in the use of invasive diagnostic and therapeutic procedures by almost every medical and surgical speciality. The data on vascular trauma from "civilian" experiences are becoming dominated by injuries of iatrogenic cause. Even were it not for medical-legal liability, the importance of prompt recognition and correct treatment of injuries that we ourselves cause is obvious, as is the need for preventive measures to be clearly identified and adopted. This book serves these needs well through a nicely balanced focus on prevention, on the one hand, with its comprehensive review of epidemiology and etiology, and on management, on the other, with its practical comments on diagnosis, treatment and outcome. The organization of this book makes it very usable. After chapters on both arterial and venous catheterization injuries, there follows a thorough analysis of injuries associated with percutaneous transluminal angioplasty and other endovascular procedures. Then, after a chapter on noninvasive vascular injuries, there follows a series of chapters dealing with vascular injuries associated with the practice of specific specialties: radiation therapy, orthopedics, neurosurgery (especially lumbar disc surgery), gynecology, head and neck surgery, urology, adult general surgery, and pediatric surgery.
This 16th volume of Advances in Neurosurgery contains a selection of pa pers presented at the 38th Annual Meeting of the German Society of Neurosurgery, held in Munster on 3-6 May 1987. The program committee had to choose these contributions from a total of 161 presentations. I am very much obliged to the members of the committee who contributed their experience and effort to the organization of the scientific pro gram: Prof. Dr. R.A. Frowein, Prof. Dr. W.J. Bock, Prof. Dr. E. Kazner, Prof. Dr. G. Lausberg, Prof. Dr. M. Klinger, and Prof. Dr. M. Brandt. The aim of the Meeting was to exchange experiences regarding modern diagnostic and therapeutic achievements. The first main topic was new technical methods in neurosurgery. Refinements in laser technology were introduced; different laser devices have been recognized as use ful tools in microsurgery and some peripheral nerve problems. The Ca vitron ultrasonic aspirator has proven its value in a great variety of intracranial and spinal tumors within a few years. Its technical advance is impressively demonstrated by some of the papers in this book. With growing experience we have recognized the great value of these additional instruments in limited surgical fields and learned not to overestimate them in the hands of a skilled neurosurgeon."

Medical information systems such as Radiology Management Information Systems (RIS), Picture Archiving and Communications (PACS) and Hospital Information Systems (HIS) will soon be standard tools to support routine work in hospitals. An interface between PACS/RIS and RIS/HIS is increasingly necessary in order to co-ordinate the flow of information throughout these systems. This book discusses a systematic analysis of interfacing strategies. An introduction is given to the status of present radiology departments and trends for the future. Then, to define a PACS-RIS interface in a multivendor environment, the so-called Marburg Model is described: a comprehensive systems analysis method that includes the requirements of radiologists, software and hardware engineers, and medical informatitians. A detailed PACS-RIS interface for a specific systems implementation is derived using the Marburg Model, which can be used as a standardized approach to designing interfaces.
In the past two decades, significant advances in magnetic resonance microscopy (MRM) have been made possible by a combination of higher magnetic fields and more robust data acquisition technologies. This technical progress has enabled a shift in MRM applications from basic anatomical investigations to dynamic and functional studies, boosting the use of MRM in biological and life sciences. This book provides a simple introduction to MRM emphasizing practical aspects relevant to high magnetic fields. It focuses on biological applications and presents a number of selected examples of neuroscience applications. The text is mainly intended for those who are beginning research in the field of MRM or are planning to incorporate high-resolution MRI in their neuroscience studies.
Osteoarticular pathology is a very frequent motive for consultation. Very often, the diagnosis relies upon symptomatology, and the physi cian requires confirmatory radiological investigations. Whatever the clinical indication, the interpretation of radiological data must be very rigorous. On the basis of a complete description of the radiographic images, according to a systematic analysis plan, a certain number of diagnostic hypotheses may be proposed. I am proud that this study of the hand appears in the series Exercises in Radiological Diagnosis founded by A. WACKEN HEIM. From time eternal, man has tried to explain the numerous configurations and lines of the hand in order to reveal the true character of a person and to display his life. The importance of the symbolism of the hand is reflected in its role in the cultural life of the old world. Therefore, it is not astanishing that the first book about the hand, by Johannes Hartlieb, was devoted to Chiromancy, and that it was so successful that it went through four editions since the second half of the fifteenth century. When Wilhelm Rontgen in 1895 asked his wife to lay her hand on a photographic plate covered with black paper and exposed it to the radiation he had just discovered, it marked the beginning of radiology. Today the study of the hand concerns only 2%-4% of the activity of radiology. It nevertheless reveals an amazing quantity and variety of pathology. The last decade has seen a rapid expansion in the range and sophistication of diagnostic imaging modalities which are available to clinicians. Our objective has been to produce a manual on paediatric radiology which will prove of value to those clinicians and radiologists in training who are preparing for the Fellowship, Membership and Diploma examinations of various colleges. This teaching manual presents radiographs and examples of other imaging modalities from 100 paediatric patients. The material was taken from a radiological teaching collection obtained from patients at The Hospital for Sick Children, Great Ormond Street, over a lO-year period by one of the authors (C.M.H.). With each case a short clinical history is given and a series of questions posed, similar to those encountered in various postgraduate medical examinations. Sample answers with com ments and more illustrations are presented on the following page. It has been impossible to achieve comprehensive coverage of the subject in a book of this size, but we have tried to select examples of those cases which illustrate the range of imaging modalities currently available and which may be encountered both in clinical practice and in examinations. We acknowledge with gratitude the kind assistance of Miss Sugarhood in the preparation of the manuscript."
Marcelle Megret has assembled a comprehensive collection of radiographie images during her work in the field of com puted tomography of the cranial skeleton. This collection and her experience have made it possible for her to prepare these exercises in radiological diagnosis concerned exclusively with craniofacial tomodensitometry. The viewer will appreciate the choice of original docu ments of the highest quality, and also note the delicacy of execution of the diagrams, all drawn by the author herself. Marcelle Megret, like most of my students, conceives and carries out the schematas herself. The requirements concern ing certainty, strictness, precision, and objectivity would lead her to agree with Ludwig Wittgenstein's observations that since certain things cannot be said they must be shown, and that rules are insufficient when there are no examples. I should like this book to be successful. It inaugurates Marcelle Megret's career as an author and is an honor to the University of Geneva.
Level set methods are numerical techniques which offer remarkably powerful tools for understanding, analyzing, and computing interface motion in a host of settings. When used for medical imaging analysis and segmentation, the function assigns a label to each pixel or voxel and optimality is defined based on desired imaging properties. This often includes a detection step to extract specific objects via segmentation. This allows for the segmentation and analysis problem to be formulated and solved in a principled way based on well-established mathematical theories. Level set method is a great tool for modeling time varying medical images and enhancement of numerical computations.

This volume is the latest in the series Exercises in Radio logical Diagnosis, launched by the Strasbourg School of Radio logy. Further volumes on the facial bones, senology, and abdominal tomography are in preparation. Jean-Louis Dietemann has over a decade's experience of radiodiagnosis of the skull, and has already proven his talents as a teacher with his earlier books on the sella turcica and on carotid angiography. This book contains the contributions to the symposium "Functional Mapping of the Brain in Vascular Disorders", held at the Thirteenth World Congress of Neurology, Sep- tember 1-6, 1985 in Hamburg, FRG.
