It may have once been known as Disease of Kings, but gout is a growing problem for ordinary people around the world. This book brings together multinational expertise to draw out an understanding of this disease that will be useful for the academic, the clinician and indeed the sufferer of gout. Highlighting recent understandings of the genetic background to the condition, with detailed pathophysiology and epidemiology, this book brings together and illustrates the disease mechanisms that cause gout. There follows both an academic and practical appraisal of the dietary risk factors for the development of gout, and the cardiovascular associations that heighten morbidity. With state of the art imaging modalities, the process and sequelae of the disease is captured within the human body, describing in pictures the damage and effect that gout crystals can produce. That damage can be avoided however, if simple principles of treatment and lifestyle modifications are followed. Horizon scanning for yet more drugs to address this growing and unmet need is highlighted as new options that have either recently become available or will be accessible in the near future are explored. With the increasing incidence and prevalence of this condition we are all going to have greater exposure to gout. This book is a timely resource, written by experts, yet accessibly written for all, and certainly not just for kings.

This book presents paraproteinemia and the highly complex scientific information behind it to clinicians in a way they can understand and, ultimately, apply to their diagnostic and treatment practices. Paraproteinemia or dysproteinemia is characterized by the overproduction of an immunoglobulin by clonal expansion of cells from the B cells lineage which includes the plasma cells. The resultant monoclonal protein can be composed of the entire immunoglobulin or of its components. The identification and categorization of the different representatives of this group of disorders have traveled a long distance, but despite its high prevalence there is surprisingly limited evidence, in some areas, to guide the best clinical practice both at the time of diagnosis and during follow up. This book seeks to fill that gap. The book is divided into three sections, introductory chapters, disease entities and experimental therapies. The text guides clinicians through these diseases and disorders. The group of renal diseases attributed to deposition of monoclonal immunoglobulins or their components are arranged as one disease category in order to simplify the understanding of these complicated diseases in plasma cell dysplasia. In emulation of this practical approach, at a larger scale, the volume editors and chapter authors attempt to encompass the whole spectrum of the paraproteinemias in this text. This will have the dual benefit of offering the audience a panoramic view of this group of disorders and simultaneously keeping them focused on its individual representatives. There have been many recent advances in diagnosis, risk stratifications, and management of many members of this group of diseases and they are explored in depth here. This is an ideal guide for rheumatologists, hematologists, nephrologists, immunologists, oncologists, and relevant trainees caring for patients with paraproteinemia and related disorders.
This volume on Rarer Arthropathies, part of the series "Rare Diseases of the Immune System" aims to fill a gap in the literature by informing and updating clinicians on the unusual and uncommon forms of arthritis and related musculoskeletal conditions. It will be a valuable go-to resource for clinicians to support them in differential diagnosis, in particular when facing unusual presentations. Nineteen chapters written by experts have covered a wide range of these conditions, providing first rate information on topics yet lacking adequate coverage; from Palindromic Rheumatism to SAPHO Syndrome and other chronic immune-mediated conditions, while also including arthritis in leprosy, tuberculosis and chikungunya. Smaller, crisp paragraphs and apt subheadings will help the readers in easily understanding the up to date information provided in the chapters. Line diagrams, original full-color clinical photographs and informative tables also support easy assimilation of information, turning this book in a useful ready reckoner on the desks and smartphones. All specialists, including rheumatologists, orthopedic surgeons, pediatricians, physicians practicing internal medicine and rehabilitation medicine will find this volume invaluable in their daily clinical work.

This book has been designed to lead you through the first steps of the shoulder arthroscopy. Offering a companion guide, its numerous illustrations and videos support the text and aid in comprehension. The most common mistake in shoulder arthroscopy is to focus on the screen, whereas an external perspective is needed, though not always easy to achieve in practice. We recommend following the chronological order for the first techniques, which are then detailed one by one and can be read separately depending on the problem encountered. These techniques, all of which were developed by the author in the course of 20 years of arthroscopic surgeries, are characterized by their simplicity. After having mastered these techniques, the reader will have the basis needed to evolve on his or her own.
