With the increasing use of immune checkpoint inhibitors (ICI) across various cancers, the trends for indication at earlier stages, and the use of combination immunotherapy, the frequency of ICI-induced immune-related adverse events (irAE) is expected to grow substantially. Management of these irAE is challenging as it requires not only consideration of the toxicity but also risk-benefit ratios with respect to the primary cancer. Several rheumatic irAE have been reported with ICI therapy including arthritis, myositis, polymyalgia-like syndromes, sicca/Sjogren-like manifestations, and several other less common systemic autoimmune features commonly associated with connective tissue disease. This handbook provides clinicians with a comprehensive overview of the management of rheumatic irAE that develop from cancer immunotherapy. This book has been designed to lead you through the first steps of the shoulder arthroscopy. Offering a companion guide, its numerous illustrations and videos support the text and aid in comprehension. The most common mistake in shoulder arthroscopy is to focus on the screen, whereas an external perspective is needed, though not always easy to achieve in practice. We recommend following the chronological order for the first techniques, which are then detailed one by one and can be read separately depending on the problem encountered. These techniques, all of which were developed by the author in the course of 20 years of arthroscopic surgeries, are characterized by their simplicity. Significant progress has been acquired in the treatment of rheumatic conditions with the introduction of biologic therapies, which has enabled better control of disease activity and improved patients long-term outcome. Apart from several biologic treatments already licensed for use in autoimmune rheumatic conditions, numerous other agents are currently under investigation. This rapid expansion of the therapeutic armamentarium requires a critical analysis of individual biologic options and their clinical indications, in order to facilitate the optimal use of these new therapies.
