This book is written with occupational therapy students in mind, as a guide to newly qualified occupational therapists and for those returning to work after a break in service. Over the years I have been asked many times by newly appointed staffwhether I could recommend a book to prepare them for working with orthopaedic patients. I hope this small volume will fill the gap in the literature on the subject, and that it will be useful as a quick reference book. I hope it mayaiso fall into the hands ofthose in other disciplines and enable them to understand and appreciate the contribution of the occupational therapist to the rehabilitation team. This is the era ofjointreplacement, with ever-increasing demand forprimary and revision surgery. Improved implants and improved surgical techniques are constantly being researched. Surgery for bone tumour is less mutilating and more hopeful than ever before. Operations to release tendons and soft tissue contractures, tendon transfers, osteotomies, spinal fusion, joint fusion, etc. are performed on patients with neurological problems, thus improving function and appearance and preventing further deformity. These are some examples of procedures in this exciting and fast-developing field, while hospital beds are occupied for an ever shorter period of time and the potential for occupational therapy is enormous. 'If surgery is to be successful, the importance of assessing the patient as a human being cannot be over-emphasised' (Souter, 1987). This is precisely the approach of the occupational therapist.

Long-term memory endures and short-term memory becomes tenuous in old age. We are our memories. Linking an old person to their experience has been shown to reaffirm 'the self'. Second, it may only be possible for those who care for confused old people to understand their worries and pre-occupations if their past lives are known and respected. Optimal function of our bodies occurs when our sensory and motor systems are strong and able to adapt to the demands of the environment. Today's children often struggle with fidgeting, decreased attention, and immature motor patterns which manifest themselves through frustration and behavior issues. When our body's systems are not integrated to form a strong foundation, difficulties in many areas occur. This book provides education about using movement to improve overall function. The philosophy of normalization and promotion of the plight of children and adults with mental handicaps has drawn more public attention in recent years. Governments in a number of countries have embarked upon policies involving the dosure of institutions, move ment of people with mental handicaps back into the community, and development of community-orientated programmes, although their reasons for this may be economically, rather than ideologically, motivated. Occupational therapists have moved into the community, along with other health professionals, in order to set up community services for people with mental handicaps. My own experience of working in a multidisciplinary team in Central London for 2V2 years, helping adults with mental handicaps to move out of an institution, has been a source of motivation to write this book. The amount of written material available on the occupational therapy approach to re settlement and de institutionalization is limited, in comparison with the massive amount of information written by, and for, other practitioners. Additionally, the number of texts written by and for occupational therapists with this dient group are few. Despite excellent support from OT colleagues, I experienced considerable frustration trying to define and perform my role, not helped by a shortage of texts to draw on."

The manual contains lessons that correspond to preschool-level books that are commercially available in both Spanish and English. The lessons are presented in both languages. The program is a nine-week intervention; each week has four lessons based on bilingual fiction and nonfiction preschool books with target vocabulary words each week. The intervention uses manipulative objects to help children experience each new word and play activities to stimulate language expression. Additionally, every two weeks, are lessons that review the vocabulary and the oral language activities practiced in the previous weeks. The program has been evaluated and follows rigorous evidence-based practice guidelines.With this much-needed manual, clinicians will be equipped to provide effective intervention.
