An understanding of microbiology is essential for both medical students and newly qualified doctors, and is key knowledge for a working doctor. Learning Microbiology through Clinical Consultation introduces the subject with a wealth of life-like case scenarios, each linked with the relevant microbiology. Spanning 14 categories of infection, the cases within the book allow the reader to listen-in on clinical consultations with patients exhibiting an array of symptoms. Each case then describes the examination and the specimen taking techniques. The results of the samples taken in the story provide a clear link to the more technical information on microbiology which is then discussed. The structure of the book is laid out for easy navigation and has clear case-story headings to enable 'just in time learning' between patients, as well as 'just in case learning' when preparing for exams. Learning Microbiology through Clinical Consultation is a highly accessible text that describes the basic science of microbiology within the practice setting in an insightful and informative way. First published in 1912, French's Index of Differential Diagnosis helps clinicians in the differential diagnosis of any condition which may be seen in hospital or general practice. Arranged alphabetically by symptom, the text helps readers identify each presentation, describes the different diagnoses that it could represent, and explains the tests used to make a diagnosis. Colour clinical photographs help with diagnoses at a glance. The book is completely revised - many sections are largely rewritten, new ones added, diagnostic methods updated, many old illustrations replaced and others inserted. The emphasis, however, remains the same: the importance of a careful history, detailed clinical examination and the judicious use of laboratory and imaging investigations in the elucidation of the correct diagnosis. This reference is indispensable to medical students as well as trainee and established doctors in both general and hospital practice.

Blood transfusion is considered a life-saving therapy since ancient times, but, at the same time, a high-risk procedure. Nowadays the common perception is that infection is the greatest risk, even if the blood has never been safer from this point of view. Currently, the residual risk of transfusion must be related mainly to immunological mechanisms underlying to AB0 and minor blood systems, to compatibility of blood transfused and to development of irregular antibodies in transfused patients. "Transfusion Medicine and Patient Safety" aims to provide the basic of immunohematology to readers and to analyze the transfusional process highlighting the most critical points, thus more exposed to errors. Screening on blood and blood components for infectious diseases along with the surveillance action on emerging viruses results in the drastic reduction of post-transfusion infection, together with the potential to further increase the level of security from infection through the inactivation of blood components. The text also describes the major diagnostic systems and organizational models that modern technology provides us with a correct immunohematological diagnosis and an appropriate trasfusional therapy.
Despite diagnosis being the key feature of a physician's clinical performance, this is the first book that deals specifically with the topic. In recent years, however, considerable interest has been shown in this area and significant developments have occurred in two main areas: a) an awareness and increasing understanding of the critical role of clinical decision making in the process of diagnosis, and of the multiple factors that impact it, and b) a similar appreciation of the role of the healthcare system in supporting clinicians in their efforts to make accurate diagnoses. Although medicine has seen major gains in knowledge and technology over the last few decades, there is a consensus that the diagnostic failure rate remains in the order of 10-15%. This book provides an overview of the major issues in this area, in particular focusing on where the diagnostic process fails, and where improvements might be made.

Today most of immunochemistry methods for the determination of proteins, peptides, drugs, and many small molecules are fully automated, with good precision, excellent sensitivity and short reaction time. However, inaccuracy due to poor standardization and the presence of interfering substances in biological samples is still a serious and life-threatening issue. Proper validation of methods and quality assurance have little effect on frequency of occurrence of false positive or false negative results, which, if unrecognized, may lead to patient's misdiagnosis, unnecessary treatment or even unnecessary surgery. Deep knowledge of basic principles of immunochemical methods (antigen-antibody reaction, standardization, matrix effect, limit of detection, cross-reactivity, etc.), sources of analyte-independent interferences (preanalitycal errors, the presence of binding proteins, the presence of autoantibodies) and analyte-dependent interferences (presence of heterophilic antibodies, high-dose effect) are very important to understand, detect, reduce and/or eliminate the interferences. This book helps to reduce false results and, at the same time, improve patient's care and patient's safety.
This groundbreaking book explains prognosis from the perspective of
doctors, examining why physicians are reluctant to predict the
future, how doctors use prognosis, the symbolism it contains, and
the emotional difficulties it involves. Drawing on his experiences
as a doctor and sociologist, Nicholas Christakis interviewed scores
of physicians and searched dozens of medical textbooks and medical
school curricula for discussions of prognosis in an attempt to get
to the core of this nebulous medical issue that, despite its
importance, is only partially understood and rarely discussed.

The Handbook of Electrogastrography is the first textbook dedicated to reviewing the physiology of gastric myelectrical activity and the measurement of this electrical activity with electrodes placed on the abdominal surface - the electrogastrogram. The Handbook is divided into three major sections. The first section (Chapters 1-3) focuses on the history of electrogastrography, electrical activity of the interstitial cells of Cajal, the cells from which gastric electrical rhythmicity emanates. The cellular level of gastric electrical rhythmicity provides an understanding of the physiological basis of the electrogastrogram signal. The second major section of the book (Chapters 4-6) incorporates the practical aspects of recording a high quality electrogastrogram and approaches to the analysis of the electrogastrogram using visual inspection and computer techniques. This section focuses on the authors' combined experience of examining EGG recordings for more than sixty years. From this rich research and clinical experience, the clinical application of EGG recordings in an approach to patients with unexplained nausea and vomiting is described. Neuromuscular disorders of the stomach involving gastric dysrhythmias are reviewed. The third major section of the book (Chapters 7-9) comprises many examples of gastric dysrythmias ranging from bradygastrias to tachygastrias and mixed dysrythmias. Current understanding of the mechanisms of gastric dysrhythmias is reviewed. Artifacts in the EGG signal, which may be confused with gastric dysrhythmias, are also presented. The Handbook of Electrogastrography will be a valuable reference for physicians interested in recording gastric electrical activity in clinical practices or in clinical research. Gastroenterologists, internists, psychologists and others with an interest in gastric myoelectrical events will also find extensive and relevant information for recording and interpreting EGGs in the Handbook.

Epidermal growth factor receptor (EGFR)-mutant non-small cell lung cancer (NSCLC) is a clinically important driver alteration affecting approximately one-third of lung cancer patients. Treatments for EGFR-exon 19 deletion and exon 21 L858R NSCLC have evolved over the last decade from first-generation reversible tyrosine kinase inhibitors (TKI) to third-generation irreversible TKIs, of which osimertinib has been the widely accepted as first-line therapy. Despite survival improvement seen with osimertinib and its efficacy against acquired T790M mutation, resistance through on-target and off-target pathways eventually develop. This Element describes the structural biology and pathophysiology of EGFR-mutant NSCLC and discusses past, current, and future treatment options in the metastatic, neoadjuvant, and adjuvant settings. It describes the biology and recently approved treatment for EGFR-exon 20 insertion mutation and the treatment for the uncommon exon 18 (G719X), 20 (S768I), and 21 (L861Q) mutations. It also outlines the promising clinical applications of circulating tumor DNA (ctDNA). | You may like...
