We start life with a breath, and the process continues automatically for the rest of our lives. Since the first edition of this book in 1967, interest in sarcoidosis has increased world-wide, leading to increasing numbers of published clinical, epidemiological and laboratory studies, notably in immunology and in the pathogenesis of granulomatous inflammation. Aseries of international con ferences which started as an informal gathering in London in. 1958 has continued at approximately three-yearly intervals with increasing numbers of participants and more formal organisation, and the proceedings of all but the first have been published. When the preparation of a second edition was suggested to me, I consi dered several questions. ls there still a piace for a comprehensive, clinically orientated book on sarcoidosis? Although the reports of the international conferences bring together research reports and some reviews of topics of contemporary interest, they are useful principally to specialists in the sub ject; and, like the original papers scattered through many journals, are not convenient sources of reference for clinicians. Continued enquiries about the availability of the first edition long after it had gone out of print suggested that it had provided such a source. I therefore concluded that a second edition should be prepared, and then had to consider whether, as a physician retired from academic and hospital appointments, I was the right person to undertake the major task of incorporating in it the still-relevant parts of the first edition, the considerable amount of new information that had accumulated since 1967, and a conspectus of current studies.

Although respiratory syncytial virus (RSV) has been a high priority for vaccine development for over 50 years now, still no vaccine is available and none has yet demonstrated sufficient promise to move to licensure. The success of RSV immune prophylaxis and the availability of ever more powerful tools to study the immune response and pathogenesis of disease, combined with the ability to construct a wide variety of vaccines using different vaccine platforms, give us grounds to believe that an RSV vaccine is within reach. In 1772 in Uppsala the Swedish chemist Karl Wilhelm Scheele discovered the element Oxygen. Two hundred and one years later, in 1973, the International Society on Oxygen Transport to Tissue (ISOTT) was founded. Since then there has been an annual ISOTT meeting. After 24 years of international ISOTT meetings it was decided, at the 2005 summit in Bary, Italy, that the 2007 meeting was to be held in Uppsala, Sweden. Thus, after the Louisville meeting we, in the Uppsala group, withdrew to the Edgewater Resort at Taylorsville Lake outside Louisville and prepared the Uppsala ISOTT meeting by tasting Kentucky Bourbons, smoking cigars while bathing in a jacuzzi in the hot dark Kentucky night full of fire flies and a sky full of stars. The ISOTT program should include different aspects of oxygen - however, it is accepted that each meeting has its own local "touch". We decided to focus the Uppsala ISOTT meeting on the theme of "Imaging and measuring oxygen changes". With this in mind we invited scientists within and outside the ISOTT society. We then also received lots of good abstracts from ISOTT members that were included in the program. Lars-Olof Sundeloef introduction speech "AIR AND FIRE" concerned how oxygen was discovered in Uppsala in 1772 by Karl Wilhelm Scheele. After the introduction speech a get together event took place in the magnificent and spacious foyer of Uppsala University main building. The vice chancellor Ulf Pettersson welcomed all delegates to Sweden and Uppsala.
