Vascular Manifestations of Systemic Autoimmune Diseases (Hardcover): Ronald A. Asherson, Ricard Cervera, Douglas A. Triplett,... Vascular Manifestations of Systemic Autoimmune Diseases (Hardcover)
Ronald A. Asherson, Ricard Cervera, Douglas A. Triplett, Steven B. Abramson
R4,350 Discovery Miles 43 500 Shipped within 7 - 12 working days

Not too long ago, it was thought that inflammation of blood vessels was the sole manifestation of systemic autoimmune diseases. Today, however, we know that disorders of coagulation, injuries to both neutrophils and endothelial cells, and certain lipids and amino acids are all intricately involved in the development of vascular disease. How are all these processes linked? What strategies can we employ or develop to halt the progression of vascular damage? What are the best ways to control acute vasculitis and vasospasm therapeutically? What are the risks and benefits of various strategies for the prevention of clotting? How soon should we initiate preventive strategies that protect blood vessels from arteriosclerosis in people with acute rheumatic diseases? What should be the nature of those strategies?

Vascular Manifestations of Systemic Autoimmune Diseases provides answers to all these questions. Ronald Asherson, Richard Cervera,Steven B. Abramson, Jean-Charles Piette, and Douglas Triplett- all international experts in the field-have assembled a collection of works that provide a comprehensive analysis of vascular irregularities. Now that most individuals survive the onset of acute systemic autoimmune diseases and live for decades more, halting the long-term attack on vasculature is increasingly important.
Vascular Manifestations of Systemic Autoimmune Diseases not only describes the clinical symptoms of vascular diseases; it also covers in one volume the many mechanisms involved in the pathogenesis of these disorders and addresses the newest treatments available for them.

Clinical management and signs are the focus of this practical cardiogenetic reference for those who are involved in the care for cardiac patients with a genetic disease. With detailed discussion of the basic science of cardiogenetics in order to assist in the clinical understanding of the topic. The genetic causes of various cardiovascular diseases are explained in a concise clinical way that reinforces the current management doctrine in a practical manner. The authors will cover the principles of molecular genetics in general but also specific to cardiac diseases. They will discuss the etiology, pathogenesis, pathophysiology, clinical presentation, clinical diagnosis, molecular diagnosis and treatment of each cardiogenetic disease separately. Therapy advice, ICD indications, indications for and manner of further family investigation will all be covered, while each chapter will also contain take-home messages to reinforce the key points. The chapters reviewing the different diseases will each contain a table describing the genes involved in each. Each chapter will also contain specific illustrations, cumulatively giving a complete, practical review of each cardiogenetic disease separately. Special emphasis will be given to advice on how to diagnose and manage cardiogenetic diseases in clinical practice, which genes should be investigated and why, and the pros and cons of genetic testing. Guidelines for investigation in families with sudden cardiac death at young age will also be included. This book will be written for the general cardiologist and the clinical geneticist who is involved in cardiac patients and will provide answers to question such as: Which genes are involved and which mutations? What is the effect of the mutation at cellular level? Which genes should be tested and why? What is the value of a molecular diagnosis? Does it influence therapy? When should the first degree relatives be tested and in which way?

This book describes a wide range of interventional procedures currently employed in the treatment of cardiac diseases in women with the aim of enabling not only interventional cardiologists but also cardiologists and cardiac surgeons to refine their approach to female patients and thus improve outcomes. The book reflects the increasing awareness of gender differences with regard to coronary artery disease and structural valve heart disease. Examples include myocardial infarction, which carries a worse prognosis in women in part because they suffer from this disease mostly in the 7th decade of life, present more frequently with hemodynamic instability and later from symptom onset, and have fragile, small and tortuous coronary arteries that are more technically demanding in the context of primary percutaneous coronary intervention. Another example is symptomatic aortic stenosis, which carries a better prognosis in women who undergo trans-catheter aortic valve implantation partially due to a better annulus-prosthesis match, which leads to less residual paravalvular regurgitation, better left ventricular function and lower aggregation of comorbidities. Further, in women left ventricular response to aortic stenosis is typified by muscle hypertrophy with a small ventricular cavity. These differences are reflected in the clinical manifestations and their impact on diagnosis, treatment, and choice of tools and devices for use in interventional procedures. By explaining the at times subtle variances in disease and treatment techniques for female and male patients, this book will help practitioners select the most appropriate procedures and materials in a range of settings.

Cardiovascular MR imaging has become a robust, clinically useful mod- ity, and the rapid pace of innovation and important information it conveys have attracted many students whose goal is to become adept practitioners. In turn, many excellent textbooks have been written to aid this process. These books are necessary and useful in helping the student learn the underlying pulse sequences used in CMR, as well as the imaging findings in a variety of disorders. However, one of the difficulties inherent in learning CMR from a book is that the printed format is not the ideal medium to d- play the dynamic imaging that comprises a typical CMR case. For instance, it may be difficult to perceive focal areas of wall motion abnormality on serial static pictures, but these abnormalities are often easily seen on cine loops. One might say that trying to learn CMR solely from a standard textbook with illustrations is like trying to learn to drive by looking at snapshots obtained through the windshield of a moving car. The learner needs to see the cardiac motion and decide if it is normal or abnormal; he or she needs to be in the driver's seat. An additional limitation of the ava- able textbooks on CMR is that while they often have superb illustrations of abnormal findings, these images have been preselected.

R1,559 Discovery Miles 15 590 Shipped within 7 - 12 working days

Cardiovascular disease (CVD) is the number one killer of men and women in industrialized countries. In older age groups, CVD is also the most important cause for hospitalization, and, in many countries, it is the basis of early retirement from work. Thus, CVD is associated with enormous costs for care and loss of productivity, as well as for disabilities, pensions, etc. All this has motivated clinicians and scientists to develop and implement new methodologies and technologies to better care for patients who are hospitalized for heart disease.
Efforts to improve care in the acute phases of coronary heart disease (CHD) have been successful. During the last decade, the immediate mortality risk of a patient admitted to coronary care for a suspected myocardial infarction or other acute coronary syndrome has decreased to less than 10%. Despite these achievements, CVD continues to represent a major threat to the health of middle-aged and elderly men and women.
This volume addresses myriad aspects of CHD prevention, including biobehavioral and psychosocial factors, behavioral epidemiology, behavioral intervention models, and policy. The first section of the text provides an introduction to CVD prevention and behavioral medicine. The second section introduces two theoretically different approaches to preventive action, high-risk and population-based strategies. The third section describes and discusses the important questions of how behavioral sciences can be conceptually integrated into traditional, medically based, preventive efforts. The fourth section presents both population and high-risk behavioral intervention approaches.
In summary, this volume examines the social environment and its potentials for preventive actions, reviews the psychosocial and biobehavioral mechanisms involved in these effects, and describes concrete and practical implementations of behavioral medicine knowledge as they have been applied to CHD prevention.

This book describes the most recent insights into heart failure and the role played by autonomic nervous system pathophysiology in it, discussing the therapeutic implications. While current therapeutic approaches are able to control the effects of excessive adrenergic activation in heart failure syndrome, the underlying abnormalities of adrenergic control remain unaltered and can still cause progression to unmanageable end-stage heart failure. New therapeutic pathways are therefore being explored with a view to developing interventions that can directly modulate adrenergic over-activity and restore a more appropriate balance in neural control of the cardiovascular system. The book opens by examining current heart failure therapies. Advances in our understanding of autonomic regulation/dysregulation in heart failure are then discussed in detail, in the context of the search for more effective therapies. A concluding section addresses the role of autonomic nervous system denervation in heart failure. The authors are top scientists from leading research centers.
