Tropical diseases are not merely a group of nosologic diseases indigenous to the intertropical zone. Many are diseases of poor public health originating from poverty, ignorance, and population upheaval. Tropical dermatoses represent a public health problem in 127 countries with a population of 3 billion people who do not have access to health care. Currently, infectious and parasitic diseases along with emergent diseases such as AIDS, or old re-emergent, drug-resistant diseases constitute the majority of tropical dermatoses. This handbook presents the geographical distribution, etiology, clinical picture, and treatment of dermatoses in the tropics.

It has been known for many years that the Malassezia yeasts are associated with a n- ber of different human diseases ranging from pityriasis versicolor to seborrhoeic d- matitis. However the evolving history of their taxonomy and pathogenicity, and the management of the diseases that they cause has been a long, and often diffcult, jou- ney. Their fastidious growth requirements defed the initial attempts to culture these organisms on laboratory media and their true identifcation and the relationship between different species only became apparent with the application of modern molecular techniques. Likewise although recognised in the 19th century as potential causes of human infection, piecing together the complex and, in certain cases, still uncertain relationships to different human diseases has taken many years. Recognised initially as causes of infection of the skin, they are now known to be superfcial c- mensals as well as potential causes of infections in domestic animals and more serious human conditions such as fungemia. They have also been implicated in the pathog- esis of allergic and other infammatory diseases. Given this complex, yet fascinating, history it seems appropriate to bring together current thought on these yeasts, their structure and function and their association with both human and animal disease states. The goal of this volume is to summarize the state-of-the-art in the utilization of computer vision techniques in the diagnosis of skin cancer. Malignant melanoma is one of the most rapidly increasing cancers in the world. Early diagnosis is particularly important since melanoma can be cured with a simple excision if detected early. In recent years, dermoscopy has proved valuable in visualizing the morphological structures in pigmented lesions. However, it has also been shown that dermoscopy is difficult to learn and subjective. Newer technologies such as infrared imaging, multispectral imaging, and confocal microscopy, have recently come to the forefront in providing greater diagnostic accuracy. These imaging technologies presented in this book can serve as an adjunct to physicians and provide automated skin cancer screening. Although computerized techniques cannot as yet provide a definitive diagnosis, they can be used to improve biopsy decision-making as well as early melanoma detection, especially for patients with multiple atypical nevi.

We have endeavoured in this book to bring the reader up-to-date information, which will contribute to an enhanced ability to diagnose and treat mycobacterial skin diseases. This volume is mainly concerned with conveying to dermatologists, microbiologists, immunologists, dermatologists in training, registrars in other specialties and interested family practitioners, a concise appraisal of some recent developments in the various aspects of myco bacterial skin diseases. This book focuses on neglected skin diseases and conditions in resource-poor countries through the lens of livelihoods. These diseases, when neglected, not only affect the skin, but also joint movement and physical appearance, severely influencing the lives and livelihoods of individuals and families in vulnerable communities. The book explores why neglected skin diseases thrive in resource-poor countries, and considers how this cycle might be broken by looking at the impact on livelihoods and access to healthcare in affected communities. The research is shaped by three guiding questions: Firstly, why have some measures been successful in one part of a population and not the other? Secondly, what is unique about vulnerable populations in various parts of the world? And thirdly, can the cycle of debilitating skin diseases and conditions in resource-poor countries be broken? In addressing these concerns, the book excavates past and present efforts in public health measures to combat these diseases, considers the impact of culture and livelihoods, and reviews some of the milestones in disease elimination and eradication. Basic research on the pharmacology of itch has exploded in the wake of two very influential papers that were published in Nature (2007) and Science (2009). Long overlooked as a milder form of pain, itching has rapidly gained a new appreciation in both research and clinical communities because of its complexity and its negative effects on the quality of life of the distressed patients. Like pain, not all itches are the same. Unlike pain, there are no standard drugs equivalent to aspirin and morphine. Epidemiological studies emphasize the high incidence and economic costs of itch (pruritus). It is the most prevalent symptom of a wide variety of allergic and inflammatory skin conditions (e.g., psoriasis, atopic dermatitis), is associated with several systemic diseases (e.g., chronic kidney and liver disease), and occurs in patients undergoing hemodialysis, spinal administration of opioids, and in those suffering from AIDS. The reader will learn about the multiple pathways for itch and their interactions with pain. The relationship between these closely related, yet distinct sensory phenomena, will be emphasized. Both itch and pain use several common molecules to send signals to the brain. Thus, drugs that have been, and are being, developed as analgesics may also attenuate intractable itch. This has been an exciting and very necessary turn of events since traditional H-1 receptor antagonists are ineffective in blocking the pruritus associated with kidney failure and cholestasis. The clinical chapters will provide insights into contemporary treatment regimens for pruritus in different human scenarios.
