The estimated prevalence of erectile dysfunction (ED) ranges from 15 - 50 million men in the United States alone, with all age groups of men affected. Despite still-existing taboos surrounding male impotence, more men are seeking help and treatment with the advent of newer treatment options in recent years, most notably oral PDE-5 inhibitors that shed light on the primary vascular cause and treatment target in most cases. In the majority of men, ED has a physical cause and can be effectively treated. Increased knowledge of cause and treatment options, however, requires clinicians to become acquainted with side effects of drugs, alternative treatments, new research in the field, and potential disease conditions that might co-exist with ED. Part of the Oxford American Urology Library, this practical manual illuminates the pathophysiology of erectile dysfunction and approaches to diagnostic evaluation and therapeutic options, including specifics of ED in men with chronic diseases such as diabetes, hypertension, coronary artery disease, heart failure, and chronic kidney disease. In all varieties of organ transplants, early results have dramatically improved over the past two decades and failures due to acute rejection are becoming rarer. Efficient immunosuppressive regimens have been developed with the objective of very good results at 1, 3 and 5 years. Successful transplants, however, are significantly less frequent at 10 and 20 years, and many patients require retransplantation. Many factors are involved in late graft loss and it is now well recognized that, in addition to chronic rejection, a number of non-immunologic factors play a prominent role. In the case of renal transplantation, a reduced mass loss (transplantation of a single kidney, sometimes from an aged donor, ischemic injury and alteration of some nephrons in the case of early acute rejection) will result in slowly progressing chronic renal failure, even in the absence of any supplementary attack of an immunological nature. The new treatments must be analyzed in the light of their capacity to reduce these late failures. Several preventive measures can also limit both immunologic and non-immunologic factors of late transplant deterioration.

Urolithiasis is not only one of the most frequently encountered diseases at uro logical clinics; it is also the disorder whose treatment has shown the most rapid progress in the past decade. In that period, medicine has experienced a real revolution, characterized by minimally invasive treatments, improvement of the quality of life, and cost-effectiveness in treatment outcomes. In urology, the revolution started with the development of endoscopic retrograde treatment of urolithiasis in the upper urinary tract, which led to development of the percuta neous antegrade maneuver in the latter half of the 1970s. The most remarkable event occurred in 1982, when clinical use of extracorporeal shock wave lithotripsy was introduced by the Munich group, represented by Dr. Christian Chaussy, at the 18th Congress of the International Society of Urology in San Francisco. With the advent of these new strategies, open surgery for urolithiasis has all but dis appeared. Today, with the availability of new technology and equipment, guide lines for the treatment of urolithiasis have changed in all developed countries. It is quite timely that the Meeting of International Consultation on Urolithiasis will be held in Paris in June 2001 to establish international guidelines for urolithiasis. Looking through this textbook for urolithiasis, I was greatly impressed to learn that we have already drawn up some guidelines. The book includes all the updated advances of urolithiasis presented by the most prominent and experi enced urologists from all around the world.

The aim of Bladder Research Congress, San Francisco, California, April 23-25, 1998, was to provide a forum for authoritative investigators who are actively involved in the various disciplines which define the leading edges of bladder research. Each session was introduced by a moderator followed by five to six invited expert speakers with time for extensive interaction from the participants.
