This book provides a state of the art overview of all aspects of testicular descent and cryptorchidism, including the mechanisms of descent and the causes, consequences, diagnosis, and treatment of undescended testis. The advances in understanding that have been achieved over the past two decades are clearly explained, covering the latest genetic information on the causes of normal and abnormal testicular descent, the role of INSL3 in transabdominal migration, and the evidence that a neurotransmitter released from the genitofemoral nerve mediates androgenic control of inguinoscrotal descent. Exciting changes in the management of both congenital and acquired cryptorchidism, such as the widespread use of laparoscopy for impalpable testes inside the abdominal cavity, are fully described. Evidence for the benefits of orchidopexy within the first year of life is reviewed, and an individual chapter is also devoted to hormonal treatment. Throughout the book due attention is given to ongoing controversies and divergences of opinion. This new edition of Descent of the Testis will be a timely update and valuable reference for all who are involved in research into testicular descent and management of cryptorchidism.

In the last few years, there has been a significant volume of research published in the field of urinary dysfunction. Urinary incontinence can affect both men and women of all age-groups with varying degrees of severity. It is becoming an increasingly dominant health care issue with enormous economic burden and places considerable impact on the overall quality of life, impairing several areas with physical, social, emotional and sexual limitations. This up-to-date and exhaustively written textbook addresses a very difficult, underestimated, and probably inadequately considered clinical subject. This book provides comprehensive information on urinary incontinence and is primarily intended for medical practitioners, residents and medical students for daily practice. However the general public will find this book easy to read and provides ample information that is useful in understanding urinary incontinence.

The first edition laid out the foundation with laparoscopic and robotic surgery utilizing the Da Vinci SI platform. Since then, many new advances in equipment and surgical techniques are becoming more popular. This second edition expands upon laparoscopic and endoscopic techniques and robotic surgery with the use of the new Da Vinci XI platform. This book bridges the gap between the practicing community of surgeons and the surgical innovators and provides a foundation for all classic and new techniques in minimally invasive colorectal surgery. By enhancing the surgical toolbox, the surgeon is able to progress from the novice to the master. Rather than describing the entire operative procedure by an individual author, this book compares operative steps of various technical difficulties throughout different chapters, thereby allowing the surgeon to tailor surgery to patient and surgeon`s own comfort level and experience. Chapters are written by a myriad of renowned experts in the field and discuss the major advances in advanced laparoscopic and endoscopic, robotic, and transanal minimally invasive surgical techniques. Great emphasis is placed on transanal total mesorectal excision (TaTME), which is dramatically changing the surgical approach to rectal resections. The second edition of Advanced Techniques in Minimally Invasive and Robotic Colorectal Surgery serves as a valuable resource to general surgeons, colon and rectal surgeons, minimally invasive surgeons, as well as residents and fellows.

Hormonal Therapy for Male Sexual Dysfunction (Hardcover, New): Mario Maggi Hormonal Therapy for Male Sexual Dysfunction (Hardcover, New)

Hormonal therapy offers a potentially powerful approach to the treatment of sexual disorders in men. However the interplay of different hormones within different bodily systems is carefully balanced; judicious judgement is required in the therapeutic use of hormones to minimize unwelcome side effects.

A better understanding of how hormones work will help the physician to better tailor therapies for improved sexual responses in men. "Hormonal Therapy for Male Sexual Dysfunction" will provide the rationale for hormonal therapy in male sexual disorders, explaining the language of sexual endocrinology and its application in clinical practice.

The aim of this book is to provide a clinical 'in the office' or 'at the bedside' guide to effective patient care for sexual medicine physicians, urologists, gynecologists and other health-care providers in practice and in training. The tone will be practical, not academic. The working assumption is that readers want to know what (and what not) might or should be done, without over emphasis on the why. That said, it is important to review the crucial basic science necessary for effective diagnosis and management, and to provide reminders in the context of the practical chapters.

It will not be heavily referenced, in line with a more practical approach. This allows for smoother reading (and also relieves the burden of comprehensive citing from authors). Key evidence (clinical trials, Cochrane or other meta analyses) should be summarized in 'Evidence at a Glance' boxes and key references such as reviews, major papers can be provided in the 'selected bibliography' at the end of each chapter.

Practical guidance will be provided through:

- the use of algorithms and guidelines where they are appropriate

- 'Tips and Tricks' boxes - hints on improving outcomes perhaps via practical technique, patient questioning etc

- 'Caution' warning boxes - hints on avoiding problems, perhaps via contraindications

- 'Science Revisited' - quick reminder of the basic science principles necessary for understanding

Incontinence affects four out of 10 women, about one out of 10 men, and about 17% of children below age 15. The most common reason for admitting a family member to a nursing home is the family's inability to cope with incontinence. It is estimated that in the US incontinence costs 26 billion dollars a year. While it is common to exercise many parts of the body to stay fit, very little attention is paid to exercising the pelvic floor. Many exercises exist for the general fitness of the body, strengthening of the arms and legs and the abdominal and back muscles. Finding fun exercises for the pelvic floor involves searching through a great deal of literature; finding exercises suitable for men, women, and children that are fun and effective appears to be impossible. This book should provide therapists with treatment ideas and encourage patients to ask for help. It focuses on exercises and discusses other treatment options.

Overactive Bladder Syndrome (OAB) is a clinical entity characterised by urgency, with or without increased urinary frequency and/or urgency incontinence. It is a chronic condition with a high prevalence that increases with age, thus being a growing health problem especially in developed countries due to increased life expectancy. Patients with this disorder are often reluctant to seek advice due to embarassment, fear of social exclusion or the belief that symptoms are an expected consequence of ageing. Costs associated with treatments, use of pads/diapers and general economic burden are expected to increase in the future. OAB has a negative impact on quality of life (QoL) in varying degrees, depending upon the severity of symptoms and the presence of urgency incontinence. It has been linked to increased risk of fractures, institutionalisation and depression in older populations particularly prone to the development of OAB. This books discusses the prevalence, risk factors and management options available for OAB.

This book is intended for urologists and pathologists who practice in an office setting. In this setting, bladder and prostate diseases are most commonly encountered. Therefore, bladder and prostate biopsy and urine cytology specimens are frequently obtained by urologists, which require pathologists assistance to interpret. In the office setting, communication between urologists and pathologists becomes feasible, which is paramount for accurate diagnosis and better patient management. For this purpose, this book includes clinically relevant information for pathologists, concise pathology terminology, and diagnostic tests for urologists. This book describes the common clinical and pathological entities, and is written in a bullet format for easy and quick reference. For visual review, over 150 diagrams, histologic images, and other images are included. The first and second chapters describe bladder and prostate diseases, focusing on clinical-pathological correlation. Therefore, this book is also useful for pathology and urology residents in training, in preparation for board examination(s), or as a refresher reference for recertification. The third chapter is dedicated to office-based pathology laboratory services, which outlines laboratory instrumentation, procedures, and regulatory requirements. This information is useful for urologists and pathologists who are involved in the management of an office-based pathology laboratory. Finally, I greatly appreciate Josephine Wu DDS, Esq. and Miao Cui MD, MS, who contributed to this excellent third chapter.

Acute kidney injury (AKI), defined as an abrupt decrease in renal function over a period of hours to days, is a common complication among hospitalised patients with different acute diseases. Its incidence has been increasing in recent years and is reported to be very high especially in the acute settings. Since clinical signs and symptoms of acute renal damage are not specific, it is difficult to promptly distinguish AKI at the time of patient presentation. Currently the diagnosis of AKI requires serial assessment of laboratory tests over a period of several days, and is based mainly on the evaluation of serum creatinine (sCr) and decrease in urine output as supported by Risk, Injury, Failure, Loss, and End-Stage Kidney Disease (RIFLE) criteria, Acute Kidney Injury Network (AKIN) criteria, and the recent Kidney Disease: Improving Global Outcomes (KDIGO) practice guidelines for AKI. Such a need for repeated sCr evaluations and monitoring of urinary output for too long time after admission could therefore result in a diagnostic delay. With delays in diagnosis, clinicians miss opportunities to start appropriate treatment to minimise damage, and patients incur more severe AKI with subsequent greater risk of developing progression of renal damage leading to chronic kidney disease (CKD), dialysis and increased risk of severe cardiovascular diseases and death.

This book offers gynecologists and urogynecologists a pocket guide to urodynamic testing in women with lower urinary tract symptoms. In female urology, the main indications for urodynamics are urinary incontinence, pelvic organ prolapse, urgency-frequency syndrome (also known as overactive bladder), emptying disorders and painful bladder. By collecting quantitative measurements while reproducing the patient's voiding symptoms where appropriate and possible, urodynamics helps to identify the cause of symptoms, supports management and facilitates prognoses. However, despite the undeniable advantages, the role of urodynamics in female urology is hotly debated. The book starts by describing the physiological background and methodological considerations for urodynamic best practice in the preclinical setting. It then explains how to perform urodynamics correctly - so-called "good urodynamic practice" - to increase the readers' confidence and ability to perform and interpret urodynamic investigations. Lastly, it reviews the advantages and disadvantages of urodynamic testing in each of the key clinical female urology conditions. A separate chapter focuses on the peculiarities of diagnosis and management for female patients with neurogenic bladder. This guide offers a valuable tool for gynecologists, urologists and urogynecologists, as well as a ready source of information for all physicians involved in diagnosing and treating lower urinary tract disorders in women.
