Das Harnblasenkarzinom gehoert weltweit zu den haufigsten Krebserkrankungen und wird meist erst im Endstadium erkannt. Gegenwartig verwendete diagnostische Verfahren sind invasiv, zeit- und kostenintensiv und verbunden mit dem Risiko weiterer Komplikationen. Die Entwicklung von nicht-invasiven Tumormarkern fur die Diagnose und Nachsorge von Harnblasenkarzinomen ist von grossem klinischen Interesse und koennte anhand von RNA-Markern im Patientenurin realisiert werden. Im ersten Schritt wurde daher das Transkriptom vom Urin gesunder Patienten mit dem von Harnblasenkarzinom-Patienten im fortgeschrittenen Tumorstadium verglichen und potentielle Tumormarker identifiziert. Anschliessend wurde der Fokus auf die Analyse von linearen und zirkularen RNA-Spezies des TRAM1- und S100A6-Genlokus gelegt und umfassende Studien zur Detektion und Verifizierung der Transkripte durchgefuhrt. Dabei empfahlen sich insbesondere S100A6-RNAs durch eine solide Nachweisbarkeit im Patientenurin und eine erhoehte Expression mit fortschreitendem Krankheitsstadium als Tumormarker fur das Harnblasenkarzinom. Zirkulare RNAs standen aufgrund ihrer vermuteten erhoehten Stabilitat in Koerperflussigkeiten und der bereits bekannten Tumor-biologischen Funktionen ebenfalls im Mittelpunkt der Untersuchungen. Ein besseres Verstandnis der vielfaltigen Funktionen von zirkularen Transkripten koennte zudem die Entwicklung von therapeutischen Strategien zur Behandlung von Blasentumoren vorantreiben.

London Jack Tinker 30 March 1988 Preface The first oxalate workshop was held in London in 1979 and the proceedings published privately by the Wellcome Foundation. At that time the importance of urinary oxalate as a risk factor more important for calcium oxalate stone formation than urinary calcium had been recognized. Nevertheless measurements of urinary oxalate still left a lot to be desired and in particular the non-enzymatic conversion of ascorbate to oxalate had not been rediscovered so that many measurements must have been wrong. Plasma oxalate was still difficult or impossible to measure by any reasonable, accessible methods and consequently there was still much argument and speculation about the handling of oxalate by the kidneys. A lot of work has been performed in the last eight years on oxalate metabolism and it therefore seemed to the organisers to be a good time to hold a second oxalate workshop.

Dies gilt insbesondere fur Erkrankungen, die mit einem hohen Leidensdruck verbunden sind. Hierzu zahlt auch die Harninkontinenz - eine Erkrankung, die uberwiegend Frauen betrifft und mit vielfaltigen Einschrankungen assoziiert ist. Wichtige PROMs in der Harninkontinenzversorgung sind der Questionnaire for Urinary Incontinence Diagnosis (QUID), der bei der Abgrenzung zwischen Belastungs- und Dranginkontinenz - den beiden haufigsten Inkontinenzformen - unterstutzt, sowie der Incontinence Severity Index (ISI), der auf eine Beurteilung des Schweregrads fokussiert. Gegenstand dieses Buchs ist eine Studie unter 161 inkontinenten Frauen, in der QUID und ISI erstmals ins Deutsche ubersetzt und evaluiert wurden. Im Zentrum stand die Untersuchung klassischer psychometrischer Gutekriterien wie Sensitivitat, Spezifitat, Reliabilitat und Validitat. Im Ergebnis kann der Einsatz beider Frageboegen empfohlen werden. Die massige Spezifitat des QUID bei der Unterscheidung zwischen Belastungs- und Dranginkontinenz ist jedoch zu berucksichtigen.

Robotics is one of the hottest topics in medicine today, with an international interest that is exponentially growing. The introduction of robotic technology into modern operating theatres has provoked a revolutionary change in the basic surgical approach, with many advantages over traditional open surgical treatment, including faster recovery and a significantly lower risk of surgical trauma. While the benefits of minimally invasive surgery are apparent, the expansion of laparoscopic surgery throughout the field has been relatively slow due to the steep learning curve and the level of practice and specialism required to perform such procedures. Although revolutionary upon conception, standard laparoscopy involves the surgeon working from monitors with no depth perception and also with a surgical motion that is counter-intuitive. The introduction of robotic technology however, has surpassed the traditional laparoscopic approach by providing full three dimensional vision, intuitive motion and wristed instrumentation with motion scaling. These dramatic innovations have broadened the scope of surgeons that can now perform complex laparoscopy, and while still in its infancy, robotic assisted surgery has begun to infiltrate all fields of surgery. However, while the practical adoption of the techniques and procedures has increased over the last 5 years, the educational resources have not, leaving the only available learning tools as videos, case observation and proctorships. There is therefore a severe market void for such a publication as this, with steadily growing sales around the world of robotic surgical systems. A compact book, overseen by such a respected figure and featuring contributions from the field leaders, is sure to be very successful within the next few years.

Circumcision remains as one of the most controversial topics in current urological practice. The most important argument against circumcision is the permanent change of anatomy, histology and function of the penis, with potential complications, with rates reported to be low in developed countries, whereas it may be up to 85% when circumcision is carried out by traditional circumcisers, rather than by medically trained professionals in developing countries. In some studies, reporting the complications of circumcision, primary haemorrhage was the most common (52%), whereas infection, meatal stenosis, incomplete circumcision, penile oedema, glandular injury, penile adhesions, iatrogenic hypospadias and urethral injuries were also detected at different rates. At times minor complications after circumcision which cannot be avoided even when the procedure is undertaken by specialized pediatric surgeons or urologist, in a properly equipped centres; specially if the child or his penis is congenitally abnormal, the obvious examples are, circumcising a child with an excessive suprapubic fat or a child with webbed penis or microphallus . This title aims to minimize complications of MC and to compete against its serious impact on men's' health, it will educate and teach physicians about potential complications and how they could manage it early on and avoid further patient problems This title will shed some light over the common as well the uncommon complications, which usually raise a debate about its management. Since the last edition of the Tumors of Prostate Gland, Seminal Vesicles, Penis, and Scrotum, there have been numerous significant developments in the diagnosis and treatment of prostate and penile cancers.
