Bei metabolisch-aktiver Konkrementbildung der Nieren (Hypercalciurie und Harnsaure- lithiasis) finden sich im Vergleich zu klinisch Gesunden eine Reihe von diskreten Unterschie- den beziiglich der renalen Saureelimination. Unter allopurinolbedingter Harnsauresuppression sieht man einen Abfall der Titrationsaziditat, und bei Hypercalciurie findet sich deutlich ver- mindertes Urinzitrat. Bei primarem Hyperparathyreoidismus sind diese Symptome starker ausgepragt, wiihrend Patienten mit Harnsaurelithiasis offenbar anderen Gesetzmiilligkeiten unterliegen, die vorwiegend iiber den Kohlenhydrat-und Fettstoffwechsel der meist adiposen Patienten gesteuert werden. Koch gilt die Durchblutungsstorung der Niere als wesentliches Merkrnal einer Steinentstehungskrise. Bekanntlich kann man aus den Ergebnissen der kon- ventionellen Clearanceuntersuchungen gewisse Rtickschltisse auf die Nierendurch- blutung ziehen. Ich mochte deshalb zuniichst Ergebnisse seitengetrennter Clearance- untersuchungen bei Patienten mit einseitiger Urolithiasis mitteilen. Methodik und Durchflihrung seitengetrennter Clearanceuntersuchungen kann in diesem Kreis als bekannt vorausgesetzt werden. Zur Methodik mochte ich nur so viel sagen, daB wir die PAH-und Inulin-Clearance im sog. stady state bestimmen.

Harnstauungsnieren sind die Folge zahlreicher urologischer Krankheitsbilder mit AbfluGstorungen. Ihre Funktion, die Zeitspanne bis zur Entwicklung einer funktionslosen Wassersackniere, bzw. ihre Erholungsfahigkeit, interessieren seit langem den klinisch und praktisch tatigen Urologen. 1m internationalen Schrifttum liegen zahlreiche Veroffentlichungen iiber diese Probleme vor. Quantitative funktionelle Verlaufsstudien der Harnstauungsnieren mit kom- pletter Ureterblockade waren bisher nur tierexperimentell mit erheblichem in- strumentellen Aufwand durchfiihrbar. Die von OBERHAGSEN entwickelte seitengetrennte Bestimmungsmethode des effektiven Nierenplasmastroms mit Radioisotopen ermoglichte erstmals derartige funktionelle und rontgenologische Vergleichsuntersuchungen in der Klinik. Zu- dem wurden die akuten und chronischen pathophysiologischen Veranderungen, insbesondere der Nierendurchblutung bei Harnstauung, tierexperimentell gepriift. Den pathophysiologischen Aspekten wurde eine eingehende Schilderung normal physiologischer Vorgange in der Niere sowie der klinischen Diagnostik bei Harn- stauungsnieren vorangestellt. Die vorliegende Monographie vermittelt somit einen Oberblick iiber Patho- physiologie, Diagnostik und Klinik der Harnstauungsniere. Mit der unter dem Titel "Fortschritte der Urologie und Nephrologie" heraus- gegebenen Schriftenreihe hat Professor V AHLENSIECK die Voraussetzung ge- schaffen, interessante Probleme der urologischen und nephrologischen Grund- lagenforschung einem groGeren Leserkreis zuganglich zu machen. Despite the relatively long period of time which has elapsed since its publication, that comprehensive vol- ume is still essentially valid, even though further developments have of course occurred in certain fields. In recent years the developments in nuclear medicine and ultrasonic techniques have led to a number of new methods of medical investigation, which, in different ways, complement diagnostic radiology. Functional disorders of the urinary tract can often be detected by means of radioiso- topes. Since morphologic changes are almost always preceeded by func- tional disturbances, radionuc1ide techniques in many instances produce an earlier diagnosis than radiography. Disturbances of renal blood flow, slight ureteric obstruction, and ureteric reflux are examples of pathologic states which can be detected early by the'}' scintillation camera. Bone scans, i.e., imaging of the skeletal system using a radionuc1ide, are used extensively to diagnose bone metastases now that it has been demonstrated that such metastatic growths are identified both earlier and with greater accuracy by scintigraphy than by radiographic tech- niques.
