The fourth edition of the Oxford Handbook of Urology has been fully updated to reflect the significant advances in medical and surgical opportunities since the previous edition, including pelvic laparoscopic and robotic surgery, new drugs in incontinence and male sexual health, and new laser devices for prostate surgery. Guidelines from the European Association of Urology, the American Association of Urology, and the British Association of Urology, as well as NICE, have all been updated to reflect the best evidence-based clinical practice. Featuring both additional and expanded topics, the new edition is up to date for use in everyday urological clinical practice, and now includes recent urological controversies such as the use of vaginal mesh, to offer the newest guidance to help with doctors' consultations with the patient. The handbook is a comprehensive and concise resource, spanning the whole field of urology in an easily digestible format, including the basics of patient assessment and investigations. Providing bite-sized topics with easily navigable sections and summary diagrams and photographs, it is a key source for quick reference during clinical duties in clinics, theatre, and A&E settings, whilst providing enough information for urology doctors taking the FRCS exam. The authors have a wide range of subspecialist interests, and have welcomed a brand new co-author to ensure coverage of the needs of junior doctors. The Oxford Handbook of Urology is an invaluable resource for junior urology doctors, A&E and general surgery doctors, urology ward and theatre nurses, and medical students.

Urologists were early adaptors of robotic assisted laparoscopy, and pediatric urology similarly led in incorporating the technology into the surgical armamentarium for treating even the smallest infants. And it is in that tradition that this issue of Urologic Clinics of North America surveys the journey from the earliest pure laparoscopic procedures to the most advanced reconstruction possible today due to evolutions in technology. The issue begins with the procedure that arguably popularized the laparoscopic approach in pediatric urology-the laparoscopic orchiopexy, then posit the continued relevance of pure laparoscopy in the robotic era, before finally surveying contemporary advances in robotic-assisted pediatric urologic surgery. The issue is of relevance not only tor the novice contemplating a transition to laparoscopy or robotics but also for the seasoned minimally invasive surgeon seeking to enhance skills and augment his or her practice.

PSA screening remains highly controversial due to several important disadvantages. More PSA is produced with prostatic enlargement and in other benign conditions such as urinary tract infections. False positive tests can then lead to unnecessary diagnostic workup with invasive prostate biopsy. Another major problem with screening programs in general is overdiagnosis of cancers that would not have caused harm during the patient's lifetime. For example, many prostate cancers have a relatively indolent behavior so may not require diagnosis or treatment in a patient with limited life expectancy. All forms of prostate cancer treatment have potential urinary and sexual side effects, so reducing overdiagnosis and overtreatment are critical public health issues. Because screening has many proven benefits but also significant harms, there are widely disparate guidelines on prostate cancer screening from major organizations worldwide. This issue of the Urologic Clinics will provide insights into the many different prostate cancer guidelines and related policy issues.
