Myocardial tissue engineering (MTE), a strategy that uses materials or material/cell constructs to prolong patients' life after cardiac damage by supporting or restoring heart function, is continuously improving. Common MTE strategies include an engineered vehicle', which may be a porous scaffold or a dense substrate or patch, made of either natural or synthetic polymeric materials. The function of the substrate is to aid transportation of cells into the diseased region of the heart and support their integration. This book, which contains chapters written by leading experts in MTE, gives a complete analysis of the area and presents the latest advances in the field. The chapters cover all relevant aspects of MTE strategies, including cell sources, specific TE techniques and biomaterials used. Mitochondria have been pivotal in the development of some of the most important ideas in modern biology. Since the discovery that the organelle has its own DNA and specific mutations were found in association with neuromuscular and cardiovascular diseases and with aging, an extraordi-nary number of publications have followed, and the term mitochondrial medicine was coined. Furthermore, our understanding of the multiple roles that mitochondria play in cardiac cell homeostasis opened the door for intensive experimentation to understand the pathogenesis and to find new treatments for cardiovascular diseases. Besides its role in adenosine triphosphate generation, mitochondria regu-late a complex network of cellular interactions, involving (1) generation and detoxification of reactive oxygen species, including superoxide anion, hy-drogen peroxide, and hydroxyl radical; (2) maintenance of the antioxidant glutathione in a reduced state and adequate level of mitochondrial matrix superoxide dismutase; (3) cytoplasmic calcium homeostasis, particularly under conditions of cellular calcium loading; (4) transport of metabolites between cytoplasm and matrix; (5) both programmed (apoptosis) and necrotic cell death; and (6) cell growth and development. It is therefore not surprising that this organelle has come to be the center stage in many current investigations of cardiovascular diseases, aging, and agi- related disease. Concomitant with these advances, an impressive effort is under- way for the development of new tools and methodologies to study mitochondrial structure and function, including powerful ways to visualize, monitor, and alter the organelle function to assess the genetic consequences of these perturbations.

The use of specific knock-in and knock-out transgenic models has resulted in an avalanche of genetic, molecular and protein-based information that, potentially, could result in an amazing new array of treatment and management options. However, the results of these studies also introduce a sometime bewildering array of redundant, overlapping and competing molecular pathways involved in both physiological and pathological responses.

This third edition is designed to provide a better basis for understanding and using animal models in the current climate of background knowledge and information. It is significantly different than the previous two editions. Chapter 1 is updated from the previous editions addressing general principles of animal selection. It also provides expanded tables of normal physiological values for easy reference. Chapter 2 covers preoperative care, pre-anesthesia, chemical restraint, and includes a significantly expanded section on pain recognition and analgesia particularly in rodents. Chapter 3 provides a summary of normal cardiovascular parameters obtained from intact, awake animals. The data have been rearranged in outline rather than the previous tabular form hopefully resulting in easier reference.

Chapter 4 addresses the techniques, problems and pitfalls of measuring cardiac function in animals. There is an emphasis on the proper use of these measurements to develop new treatment and management strategies as well as using them to study mechanisms of disease. Chapter 5 emphasizes the techniques, problems and pitfalls involved in the measurement of arterial function and ventricular/arterial coupling dynamics. Again the emphasis is on the use of these parameters to develop new treatment and management strategies and for studying the mechanisms of disease. Chapter 6 is a all new chapter dealing specifically with the problems and pitfalls inherent in using isolated heart preparations. The need for this chapter became apparent because so much information was published using obviously non-physiologic preparations. The use of both pumping and non-pumping preparations are described along with techniques necessary for using hearts from larger species where oxygen carrying capacity of the perfusate is critical. The importance of hypoxia and anoxia in the interpretation of results is discussed.

Chapter 7 focuses on the cardiovascular effects of the post-operative analgesic drugs commonly used today and how to avoid potential problems resulting from these effects when reporting experimental data. These data are also presented in outline form rather than the tabular format used in the two previous editions. Chapter 8 addresses the use of naturally occurring animal models of valvular and infectious cardiovascular disease. The information presented has been updated and expanded from the second edition. Chapter 9 examines iatrogenic models of ischemic heart disease.

Chapter 10 is new. It provides a review of iatrogenic, transgenic and naturally occurring animal models of cardiomyopathy and heart failure. Following its tradition of promoting novel areas of scienti?c discourse, the Ernst Schering Research Foundation (ESRF) hosted this workshop on chronic viral and in?ammatory cardiomyopathy. In late October 2004, scientists from Canada, Germany, the Georgian Republic, Great Britain, Italy, Japan, the Netherlands, Israel, Sweden, and the United States ga- ered in Berlin to discuss their concepts, hypotheses, and latest ?ndings on myocarditis and cardiomyopathy. This expert meeting was held in cooperation with the German - search Foundation, which in the same year had supported transregional collaborative research activities entitled "In?ammatory Cardiomyo- thy - Molecular Pathogenesis and Therapy." Organizing the workshop, our efforts strove to render tighter the network between the distinct disciplines involved in cardiomyopathy - search, building bridges between its molecular, pathogenetic, diagnostic, and therapeutic determinants. It all began as a story of a neighborhood, and we would like to express our hope that this long-term project, which will require much and - tensive cooperative work among the participants, will evolve to become a story of a good neighborhood.

Therapeutic hypothermia has emerged as a very important treatment option for patients with cardiac arrest as it provides significant protection from developing neurologic injury once the patient has been successfully resuscitated. Studies have demonstrated over 15% absolute risk reduction in death and neurologic injury using this therapy. Although hospitals and medical centers have become familiar with this important intervention it still remains greatly under utilized due to an experience and lack of resources to safely and effectively deploy such a program. The objective of this book is to educate and familiarize both providers and institutions as to how to develop and deploy and provide therapeutic hypothermia to their patients. The current knowledge for this is provided by speakers and national experts and also by literature review. There are several courses being provided on this as well throughout the US. These are good venues for people to come and see and get hands on experience, but there still needs to be a concrete book with references on how to go about getting this program started.

According to the World Health Report (2000 http:/ /www. who. int/whr), of the 55 million deaths worldwide in 1999, more than 16 million were secondary to car diovascular complications. With the prospect of world population increasing from the current level of 6 billion to 9 billion by the middle of this century, the burden of cardiac disease is going to increase astronomically. Furthermore, scientists are being challenged not only to reduce mortality, but also to improve quality of life. Thus, more than ever, intellectuals from different disciplines including biology, sociology, informatics and health care have to join forces to meet the mandate. The World Heart Congress with a focus on "Frontiers in Cardiovascular Health" held in Winnipeg during July 6-11, 2001, made a unique attempt to bring these specialists together to brainstorm and map out the course of action for cardiovascular research and health in the next century. Anytime there is a relative increase in the workload on the heart, there are adap tive myocardial as well as humoral responses. In spite of their relatively low prevalence, systemic vasculitides have been the object of intensive basic and clinical investigations over the last few years. As a consequence, important advancements have been achieved: from updated diagnostic and classification criteria and a more rational nomenclature to the recognition of an expanding spectrum of clinical manifestations and potentially devastating complications; from the recognition of the viral etiology of conditions such as HCV-related cryoglobulinemic vasculitis and HBV-associated polyarteritis nodosa to newly named variable vessel vasculitis exemplified by Behcet's disease; from single-organ vasculitis such as central nervous system vasculitis to the more recently emerging features of the IgG4-related, immune-mediated diseases that are showing remarkable clinical heterogeneity. In addition, intriguing pathogenetic hypotheses are being reported for certain chronic, relapsing vasculitides that are improving our understanding of their biology and basic pathophysiology.
