This book provides a comprehensive overview of the structural, nanotribological and nanomechanical properties of skin with and without cream treatment as a function of operating environment. The biophysics of skin as the outer layer covering human or animal body is discussed as a complex biological structure. We live today in a world densely populated by human beings living in close commu nication with one another all over the surface of the planet. Viewed from a certain distance it has the look of a single society, a community, the swarming of an intensely social species trying to figure out ways to become successfully independent. We obvi ously need, at this stage, to begin the construction of some sort of world civilization. The final worst-case for all of us has now become the destruction, by ourselves, of our species. I Although this warning is often repeated, we must not forget its paramount importance and the commitment that each sector of society has to make a world civilization possible. Tropical dermatology is a good example of an important area of our spe cialty that has never caught the proper attention of the leading centers of research in the developed countries, even though it comprehends major infec tious, parasitic, and nutritional problems of one-half of the world's land area and three-quarters of the world's population. The relevance of tropical dermatology in this extensive and overpopulated area of the globe has public health connotations that emphasize its importance. The sheer size of the problem makes it an urgent and outright need to recruit and train adequate personnel to do a proper job, and that includes not only dermatologists but paramedical personnel, nurses, and laboratory technicians.

This text evolved as a response to numerous requests to present a simplified approach to the diagnosis and management of most of the common aspects of both cosmetic and medical venous disorders. The authors, representing experienced plebologists from several different primary disciplines, have attempted to present simplistic paradigm-oriented approaches on how to thoughtfully evaluate and manage common vein issues with contemporary solutions. Several areas of phlebology were not addressed in this text. The management of vascular malformations are beyond the scope of this text and were not covered in detail. Although indications for the appropriate use of IVC filters are discussed, the details about the specific devices, their placement and removal are also beyond the scope of this text. Deep venous obstruction from prior thrombosis, venous compression by arteries such as the compression of the left common iliac vein by the right common iliac artery and pelvic venous insufficiency can now be managed with minimally invasive image guided treatments; the technical aspects of such treatments are complex and not discussed in detail in this handbook. Light-based therapies have been a major component of dermatologic practice. Historically, these treatment modalities have been mainly tailored to the treatment of patients with light skin. Principles governing use of light therapies in skin of color are less defined. However, there is a tremendous need to understand the benefits and limitations of these therapeutic options for dark-skinned patients as well. Demographic data in the United States alone indicate that the population and recipients of health care are rapidly changing with regard to skin phototype. Physicians who are involved in the delivery of care for patients with cutaneous problems that can be addressed by light treatments need to be able to fully understand the mechanisms, applications, risks, efficacy, adverse events, and other pertinent issues in considering treatment options for their patients with pigmented skin.

This book has been written to assist not only postgraduate students, but also int- ested dermatologists, general practitioners, and surgeons. During the last two decades, various surgical therapies have been used in the tre- ment of stable refractory vitiligo, but none of them became very popular because of their intrinsic drawbacks, and thus could not be adopted as a standard procedure. There has always been a need for a technique that gives best results without complications. The commonly used surgical procedures for the treatment of vitiligo have been narrated in short and their characteristic drawbacks have been pointed out. The c- lular graft technique is costly, requires a good laboratory and infrastructure, and except for being able to cover a wider vitiliginous area, the ultimate results are no better than tissue grafting technique. I used the small skin particles prepared from ultra thin or thin split thickness skin graft, the so called "microskin graft," in the tre- ment of vitiligo effectively with good expansion ratio, i.e., small donor skin to treat big vitiliginous area. This procedure covers the bene? t of cellular and tissue graft, but without their inherent drawbacks. The microskin graft technique has been illustrated with minimum necessary text to explain the steps. This technique is very simple to learn and execute. It is extremely 2 cost effective and can treat very large area up to 1,500 cm in an operative session.
