It provides an entry and exit barrier to various substances, offers effective protection against harmful substances, microorganisms and ultraviolet radiation, and is also paramount in immunology and body homeostasis, preventing dehydration and loss of essential minerals. The topical route has a number of advantages over oral and parenteral routes: no first-pass metabolism, greater convenience resulting in better treatment compliance, lower side-effect frequency, and prompt dosing cessation if required. However, the stratum corneum allows penetration of only 1 to 5% of substances applied topically, making this application route much less efficient. Lipophilic and small molecules (up to 500 Da) can cross the stratum corneum, since keratinocytes are covered by a lipid matrix, while it is virtually impossible for hydrophilic and large drugs to cross intact, normal skin, which means that passive topical delivery is restricted. To address this, techniques have been developed aimed at optimizing drug skin penetration by means of chemical, mechanical and physical methods, such as adding permeating active agents and using occlusion, iontophoresis, microneedling, lasers and intense pulsed light, and it is now possible to deliver medication to different skin layers or transdermally to the systemic circulation. Drug delivery is a hot topic in dermatology, with a Pubmed search generating 203431 articles. Dass die Ultraschalldiagnostik speziell mit hochaufloesendem Ultraschall (20MHz) bei entzundlichen Veranderungen der Haut sowie bei Hauttumoren und in der Diagnostik und Beurteilung von Lymphknoten insbesondere unter der Fragestellung ihrer Dignitat sinnvoll einzusetzen ist, hat die Methode inzwischen etabliert. In dem Buch wird das gesamte Gebiet der Ultraschalldiagnostik von Haut und Lymphknoten ausfuhrlich abgehandelt, das praktische Vorgehen wird beschrieben und anhand einer Vielzahl von Bildern und Befundinterpretationen vermittelt. Dabei wird auch auf moegliche Artefakten und Fehlermoeglichkeiten in der Interpretation von Ultraschallbildern eingegangen. Die klassische Magistralrezeptur hat eine lange Tradition und stutzt sich vorwiegend auf Empirie. In der Praxis sind nahezu 50% der durch Dermatologen verordneten Externa Magistralrezepturen. Es ist unumganglich, dass die Magistralrezeptur dem gegenwartigen Stand des wissenschaftlichen Fortschrittes angepasst wird. Obsolete Wirkstoffe mussen vermieden werden. Die Galenik soll optimalisiert werden. AEhnlich wie bei Spezialitaten soll sich der verordnende Arzt darauf verlassen koennen, dass bei empfohlenen Magistralrezepturen die pharmakologische Wirkung des Wirkstoffes in der Grundlage gesichert oder zumindest sehr wahrscheinlich ist. Externagrundlagen fur bestimmte Indikationen sollen gezielt unter wissenschaftlichen Gesichtspunkten zusammengestellt werden. Hauptanliegen des Buches sind gezielte therapeutische Empfehlungen unter diesen Gesichtspunkten.

Mit dem Melanom - so formulierte es vor wenigen lahren ein franzo sischer Kollege - habe er personlich keine Probleme. Es sei bei dieser Erkrankung immer entweder zu friih oder zu spat fUr eine Therapie. In diesem sarkastischen Statement spiegelt sich die ganze Ohn macht der A.rzte wider, welche angesichts der unberechenbaren Mela nomerkrankung oftmals jeden therapeutischen Ansatzpunkt in Frage zu stell en scheint. Doch ist es wirklich so schlecht bestellt urn den wissenschaftlichen und therapeutischen Zugang zu dieser Erkrankung? Und wenn ja, wel che Moglichkeiten stehen uns gegenwartig zur VerfUgung, urn diese Si tuation zu verbessern? Resignation ist keine onkologische Dimension. Ware sie es, so gabe es heute keine Heilungsmoglichkeiten bei Patienten mit Morbus Hodgkin oder Hodentumoren. Auch beim malignen Melanom besteht zur Resignation kein AnlaB. Retrospektiv gesehen haben sich unsere Kenntnisse dieses Tumors in den letzten lahren erheblich erweitert, auch wenn dieses therapeutisch kaum umsetzbar gewesen ist. Praven tion und Friihdiagnose trag en wesentlich dazu bei, bereits im Vorfeld der Erkrankung eine entscheidende Weichenstellung zu ermoglichen. Doch was passiert mit den Patienten, die trotz aller Bemiihungen Me tastasen entwickeln und damit eine infauste Uberlebensprognose besit zen? Arztlicher Riickzug ist sicherlich kein hilfreiches Angebot an den betroffenen Patienten und widerspricht auch der onkologischen Auf gabenstellung."
