Inflammation in itself is not to be considered as a disease . . . and in disease, where it can alter the diseased mode of action, it likewise leads to a cure; but where it cannot accomplish that solitary purpose . . . it does mischief - John Hunter, A Treatise on the Blood, ITfIlammation, and Gunshot Woundr (London, 1794)1 As we reached the millennium, we recognized the gap between our scientific knowledge of biologic processes and our more limited clinical capabilities in the care of patients. Our science is strong. Molecular biology is powerful, but our therapy to help patients is weaker and more limited. For this reason, this book focuses on the problems of multiple organ failure (MOF), multiple organ dysfunction syndrome (MODS), and systemic inflammatory response syndrome is, patients who have severe injuries; require major, (SIRS) in high-risk patients, that overwhelming operations; or have serious illnesses requiring intensive care; patients who have diseases elsewhere, in other organs or systems, that limit their capabilities to survive a new insult; and patients who are elderly or at high risk for sepsis or other complications. These are the patients who need our help. They need the advances in science, in molecular biology, immunology, pathophysiology, biochemistry, genetics, high technology, and other areas of maximum support at the bedside. These advances could potentially have the greatest impact on improving patient care.
This book is divided into 3 main sections: cysts and odontogenic tumors (lesions related to the dentition); fibro-osseous lesions and giant cell lesions, which are unique to the maxillofacial skeleton; and bone lesions, which also occur in other parts of the body. Individual diseases are described with a special emphasis on histomorphological aspects. Some lesions may pose considerable diagnostic problems due to shared microscopical features and the authors' aim is to provide reliable diagnostic guidelines for the surgical pathologist who has to deal with specimens from this area. The majority of the currently available textbooks on head and neck pathology are mainly devoted to soft tissue lesions, paying less attention to the pathology of the diseases that involve the hard tissues. Texts on general skeletal pathology cannot address this gap, as the maxillofacial skeleton differs from the bones elsewhere in the body in several respects. Therefore, the book discusses both bone disorders that are restricted to the head and neck, and diseases that also occur in the extragnathic skeleton.

The tremendous expansion of medical knowledge during the last few decades, together with the introduction of many new diagnostic techniques, has demanded such a degree of specialisation that no single individual can be conversant with all the information available. More and more emphasis, therefore, has been placed on the importance of teamwork and close collaboration between associated disciplines. The bone dysplasias of infancy represent a classical example of this concept. Only a few years ago these heritable conditions were divided into a relatively small number of entities, for many of which "atypical variants" were accepted. More recent studies have resulted in appreciation and early recognition of a large number of these disorders, thanks to co-operation between paediatricians, radiologists, geneticists and biochemists. Not only maya reasonably accurate prognosis be offered for the affected child in many instances, but, almost of greater value, genetic counselling concerning the chance of subsequent offspring being similarly affected has become available to parents. Most radiologists have little opportunity of becoming familiar with this rapidly widening field of diagnosis, so that the occasional case which may be encountered is likely to engender diagnostic difficulty. This Atlas should facilitate greatly the solution of the problem. It has been prepared by Professor CREMIN, an outstanding paediatric radiologist whose work has been known and admired by me for many years, in close collaboration with his colleague Professor BEIGHTON, a geneticist of great distinction.

This book describes both the technologies used in the discovery of melanoma biomarkers and the clinical application of these biomarkers for diagnosis and staging of disease, determination of prognosis, treatment planning, monitoring of response to therapy, identification of novel therapeutic targets and drug development. The aim of this atlas is to illustrate the wide range of histological appearances which can be seen in brea. st biopsies and mastectomy specimens. An attempt has been made to include photomicrographs showing the more usual appearances as well as some of the unusual patterns encountered in most common lesions of the breast. In addition some of the rarer forms of mammary pathology have been illustrated, sometimes extensively, where this was considered important in the differential diagnosis and where photomicrographs of good quality were available. Some of the low power photomicro graphs lack detail but have been retained as they illustrate Important microanatomical or architectural patterns. The majority of the photomicrographs are of histological preparations stained with haematoxylin and eosin, buta few special stains have been used to illustrate certain specific features. Only occasional photographs of gross specimens have been included. This is notto belittle the importance of gross appearances: indeed micro scopic examination of any tissue should always be complemented by careful naked-eye examination. A few mammographic pictures have also been included to illustrate characteristic patterns. There are no photo micrographs of cytological preparations as these were considered outside the scope of this book and excellent atlases of mammary cytology already exist. It is hoped thatthisatlaswill prove useful to surgical pathologists and others interested in the histological appearances of the breast. It is not intended as a comprehensive study of breast diseases, and details of clinical presentation and treatment have only been mentioned briefly."

It has only recently been appreciated that the immune and skeletal systems have major interactions. It is now well documented that osteoclasts, which are important cellular mediators of skeletal homeostasis, are derived from hematopoietic precursors that also give rise to immune cells. In addition, numerous cytokines that were first shown to regulate immune cell function have also been demonstrated to regulate bone cells and influence skeletal health. Conversely, products of bone cells appear critical for the engraftment of marrow in bone, the normal development of the hematopoietic and immune systems and provide niche for long-term memory B and T cells. In the past scientists involved in immune and bone cell investigations have rarely interacted in a significant way as these disciplines have developed independently and, for the most part, remain separate.
