This volume focuses on recent advances in understanding T cells as key players in antitumor immune responses, and as a result T cell-based immunotherapy is starting to transform the treatment of advanced cancers. However, despite recent successes, many patients with cancer fail to respond to these treatments. Defective migration of T cells into and within tumors is considered as an important resistance mechanism to cancer immunotherapy.The volume includes three sections. The first section covers general knowledge about T cell trafficking during a normal immune response but also during tumor development. The second section provides an in-depth description of the different obstacles that prevent T cells from migrating and contacting tumor cells. It was during the course of the eighteenth and nineteenth centuries that the problem of chronic alcohol dependence in modern society and its consequent medical effects emerged. The topic of drunkenness figures prominently in the thinking and writing of social reformers, politicians, theorists, medical practitioners, and psychiatrists. Eventually, by the mid-nineteenth century, 'alcoholism' was named as the disease of habitual drunkenness. Possibly the most important book to predict this was Trotter's Essay, written in 1804. Through case studies based on wide experience, he detailed the manifestations of alcoholism, ventured therapeutic recommendations, and squarely termed drunkenness a disease - indeed, a mental disease. Originally published in 1988 as part of the Tavistock Classics in the History of Psychiatry series, Roy Porter's Introduction to this facsimile reprint locates Trotter's work within the wider history of the evolution of the idea of alcoholism. It also examines the Essay in the context of Trotter's own life and mind - a mind preoccupied with what he saw as the degenerative tendencies of modern civilization and with the wider issues of drug dependence.

Since the legalisation of off-course cash betting in 1960, and the rise of varying forms of gambling, the British have come to be known as a nation of gamblers. Until this study was published in 1976, barely any evidence existed against which to assess the claim that gambling had become a major social problem. The authors present data drawn from area surveys carried out in Swansea, Sheffield, Wanstead and Woodford, and explore how well previous sociological theories of gambling agree with their findings, particular in connection with certain aspects of work and leisure. Examining different forms of gambling, including betting, bingo and gaming machines, the chapters consider how gambling choices vary between different social groups, and how much time and money is spent on them. A moment comes for every addict when the consequences are so great or the pain is so bad that the addict's life becomes out of control because of his or her behavior. Some are news making moments, such as the public censure when a congressman, minister, general, or professional athlete is cited for unacceptable sexual behavior. For most people those moments are followed by resolves to "never do it again," but somehow after the promise is made, they often find themselves in the exact same location doing something they vowed to never do again. That is addiction.
And for some, this addiction is more difficult to diagnose than in others. Such is the case for gay men. For some gay men fully committed to open sexual choices and experiences, modifying their sexual behavior and restricting their sexual freedoms is like going back in time and surrendering to homophobic attitudes often found in conservative culture. It just doesn't feel right. After all, the urban gay male culture surrounding him fully supports his sexual exploits as long as the sex is safe. And since gay male sex addicts may not discuss their sexual behavior even with good friends, nobody challenges them on their late-night exploits in unsafe places, their arrest record or brushes with the law, or potential health risks because of their behavior.
The truth is gay male sex addicts are not compulsively sexual because of their sexual orientation, but rather as a consequence of their individual psychological issues and biological predisposition toward addiction. This is exactly the same set of risk factors presented by straight male sex addicts.
Unfortunately, for the gay (sex) addict, his increasingly destructive patterns of behavior take place against a cultural background of dramatically greater sexual and social freedoms than those enjoyed by his heterosexual peers. The single urban gay man who has problems with sex, alcohol, or drugs is in some ways a prisoner of his own freedoms, having fewer cultural opportunities for self-examination and less cultural support for behavior change than does an average heterosexual male.
Finally, there is a resource that can help gay men determine if they are indeed a sex addict and whether or not they are willing to explore behavior change because of their sexual behavior.

In the early 1980s the transtheoretical model of change was still in its infancy. Seminal publications were just appearing, but the model already seemed to hold such promise that we made it the organizing theme for the Third International Conference on Treatment of Addictive Behaviors (ICTAB-3), which convened in Scotland in 1984. That meeting gave rise to the first edition of this volume (Miller & Heather, 1986), which focused on processes involved in moving people from one stage to the next. With the volume still in print more than a decade later, we were approached by Plenum Press with the idea of preparing this second edition. We were, obvi ously, persuaded that there was merit to the idea. Since 1986 the work of Pro chaska and DiClemente has grown exponentially in popularity and influence. In Britain and the Americas, it is now unusual to find an addiction professional who has not at least heard about the stages of change, and more sophisticated applica tions of the transtheoretical model are spreading through health care systems and well beyond. The model has influenced professional training, health care delivery, and the design of many studies including a number of large clinical trials."

Radiophannaceutical research has recently undergone a major change in direction. In past years it has been concerned mainly with the development of perfusion tracers, the biodistribution of which reflect the regional blood flow to areas of major organs such as the heart and brain. However, a major new direction of interest now lies in the development of receptor-binding radio-tracers which can be used to perform in-vivo characterisation of diseased tissues and it is likely that much of the future research in this field will follow this direction. The difficulties in developing such tracers are considerable. The researcher must first identify a promising target for radiopharmaceutical development. High specific activity radioactive molecules must be designed and synthesised which will both bind to the target receptor with high affinity, and also have the physicochemical characteristics which will allow them to reach the target site in sufficient quantity while at the same time showing minimal uptake in non-target tissues. Thus the knowledge base required for radiophannaceutical development has now expanded beyond the limits of radiopharmaceutical chemistry to include aspects of biochemistry, molecular biology and conventional drug design. The portfolio of basic knowledge required to support current radiopharmaceutical development is changing and scientists working in this arena need to be trained in this regard. At the same time, the very latest developments in the field need to be communicated to the scientific community in order to stimulate the advancement of this exciting new direction of research.
