The "VIP" is a user-friendly, point-in-time assessment tool which provides an 'at a glance' vocal profile for the clinician, voice coach and client. It examines the potential impact of a number of specific factors on the voice: general health; vocal history; vocal health; voice care; vocal status; voice genogram; anxiety and stress; social functioning; vocal demand; and environmental factors, and offers an overview of the theory to support this choice. The questions have been carefully designed to elicit easily-recorded information from the client about a range of factors that are known to affect vocal quality. The tick-box answers then translate into the Vocal Impact Profile, a visual presentation of the areas of greatest impact on voice. In addition to offering a point-in-time profile, the "VIP" may also be used as a template for achieving change in a specific area of vulnerability and provide a robust visual reinforcement of that change over time. Self-administered by the client, the questionnaire should take approximately ten to fifteen minutes to complete. The clinician or voice coach enters the responses into the computer programme (supplied on downloadable resources) or transfers them manually on to the hard copy thus offering a clear visual representation of the results. Worked examples of the VIP are included, presenting six clients with very different aetiologies and with a range of vocal problems. These examples clearly demonstrate the value of the Profile and its visual impact. As a clinical tool the "VIP" provides a subjective qualitative measure, which may be used as an adjunct to other assessment procedures. In addition, it provides a concrete method of determining and ordering the factors to be targeted in terms of therapy or voice work. Using the "VIP" should shorten the time necessary for completion of case history in the case of a voice therapy client, and, when used by a voice coach, the profile will identify areas for further discussion. The Profile is not time-sensitive so it may be repeated to evaluate client awareness of vocal change and implementation of agreed strategies. The "VIP" provides the clinician or voice coach with an efficient and effective means of auditing the therapy or coaching process and promotes a holistic partnership model of intervention.

Through analysis of a single audio-recorded and transcribed conversation, Wayne Beach reveals the altogether pervasive and often troubled talk surrounding family medical predicaments. From a careful review of extant theories that seek to explain eating disorders and grandparent caregiving, it becomes clear that an overreliance on self-report data has promoted underspecified understandings of "social contexts" -- conceptualizations void of real time practices and interactional consequences mirroring how families manage their daily affairs and understandings regarding health and illness. In contrast, this volume draws attention to family members' embodied interactional activities. Here it is seen, for example, how methods for expressing concern and caring by "individuals" may nevertheless eventuate in interactional troubles and problems "between" family members. The analysis reveals that, while displays of basic concerns for others' health and well being are routine occurrences between family members in home environments -- and of course, across friendship and various support networks -- even the delicate and well-intended management of such occasions guarantees neither agreement on the nature of the alleged "problems" nor, consequently, a commitment to seek professional help as a means of remedying a medical condition. In such cases, the very existence of an illness is itself a matter of some contention to be interactionally worked out. And it is perhaps both predictable and symptomatic that those explicitly denying (or as with the granddaughter, indirectly failing to admit) that problematic health behaviors exist, also somehow let it be made known that far too much attention is being given to possibilities and consequences of illness in the first instance.
Implications of this investigation extend well beyond "bulimia" and "grandparent caregiving" to a vast array of casual and institutional involvements between family members, friends, and bureaucratic representatives such as those involved in long-term caregiving, dealing with cancer and Alzheimer's disease, or conducting psychiatric interviews and HIV/AIDS counseling sessions. Findings regarding the interactionally organized nature of talk about bulimia, as well as the problematic nature of caregiving, will be of value to researchers focusing on language and social interaction, health practitioners, and families alike.

The Importance of Measuring Alcohol Consumption To date, alcohol studies have attended far more to issues of alco hol dependence and the harmful consequences of drinking than to the level of alcohol consumption itself. This is, perhaps, not surprising since dependence on alcohol is believed to constitute a meaningful and distinct medical syndrome, regardless of the level of alcohol consumption associated with it (Edwards and Gross, 1976). Also, of society is generally more concerned with the adverse consequences drinking (e. g., traffic fatalities, homelessness, health care and legal expenses, and academiclbehavioral problems in young drinkers), than with the quantity of alcohol actually consumed. Nevertheless, accurate assessment of alcohol usage is important in its own right in at least four contexts: 1. Evaluating the effectiveness of alcoholism and alcohol abuse treatment and prevention efforts. Such efforts include both applied evaluations of existing programs and formal, well-controlled efficacy studies on experimental interventions. These investigations require rigorous methodologies to assess outcomes precisely and contrast what may be quite subtle differences between programs and between pre treatment and posttreatment outcomes. Although these studies are usually characterized by the employment of multiple measures of success-including general improvements in social and physical functioning, reduction in degree of dependence, and resolution of problems directly resultant from drinking-it is by their assessment of changes in drinking behavior that they are potentially able to achieve the highest level of objectivity and exactitude."
