Local-regional anesthesia and pain therapy represent two areas of common interest for all physicians. A thorough preparation in functional anatomy and general pharmacology, including familiarity with local anesthetics and analgesics, is considered fundamental background for further study or clinical application of such techniques. Knowledge of the mechanisms of action, efficacies and side effects of the relevant drugs is essential, as is appreciation of possible drug interactions to evaluate eventual toxic effects. The study of both cadaveric axial cross-sections and CT scans is the basis of 21st century anatomy, and the cornerstone of clinical diagnostics. Modern medical imaging, such as CT (Computed Tomography) scans, produce 1-Dimensional anatomic cross-sections of the axial plane. Learning the proper sequence and orientation of axial cross-sections and CT scans is often extremely challenging, even for the most dedicated students of anatomy: The shapes seen in the axial plane have little relation to the more familiar coronal plane. Most texts abandon students to simply memorize the shapes seen at high-yield vertebral levels or perform tricky mental gymnastics, as they must mentally rotate the axial plane to the more familiar coronal. Students are further frustrated when learning CT scans, as the shapes seen in gray/white CT slices have little relation to the anatomic structures from which they are derived. This text serves to solve these problems by illustrating the sequence of axial cross-sections and CT scans in unique 3-Dimensional illustrations. This 3-D approach clearly demonstrates the relation of the shapes seen in cross-sections and CT's to their more familiar coronal/sagittal orientation. The illustrations themselves have been done by Dr Jackowe in the classic style of Vesalius and Bourgery, thus creating a work that is both informative and artistic, the first aesthetic anatomy textbook for many years. This text describes a system of reporting breast fine needle aspiration biopsy that uses five clearly defined categories, each described by a specific term and each with a specific risk of malignancy. The five categories are insufficient/inadequate, benign, atypical, suspicious of malignancy and malignant. Each category has a risk of malignancy and is linked to management recommendations, which include several options because it is recognized that diagnostic infrastructure, such as the availability of core needle biopsy and ultrasound guidance, vary between developed and low and middle income countries. The functional anatomy of human somatosensory cortex is of both scientific and clinical interest. Scientifically, it provides insights in information processing in the human brain. Clinically, it helps to avoid neurological deficits by sparing essential brain regions during neurosurgical procedures adjacent to central fissure. In the present study the functional organization of the human somatosensory cortex was investigated with electrophysiological techniques using a combined approach of cortical stimulations and somatosensory evoked responses on electrocorticography, scalp-EEG, and magnetoencephalography. The spatiotemporal structure of the evoked response was studiedwith biophysical modeling techniqueswhich allowed identification of the three-dimensional intracerebral location, time activity, and interaction of the neuronal sources activated following peripheral somatosensory stimulation. Furthermore, the somatotopic organization ofhand and lip somatosensory cortex was investigated. The relative value of invasive (cortical stimulations and electrocorticography) and non-invasive (scalp-EEGand magnetoencephalography) was assessed. The combined use ofscalp-EEG and magnetoencephalography was useful to increase non-invasive localization accuracy. Museie biopsy is a long-established technique in clinical practice having been introduced by Duchenne in 1868 (Arch. Gen. Med., 11, 5-179). However, the needle method used by Duchenne was not generally adopted, although Shank and Hoagland described a similar technique in 1943 (Science, 98, 592), and open muscle biopsy has for long been preferred in clinical practice, even with the advent of newer needle biopsy methods (Bergstrom, 1962, Scand. J. Clin. Lab. Invest., 14, Suppl. 68, 1-110). The development of enzyme histochemical techniques has contributed greatly to knowledge of muscle pathology. More recently electron microscopy and immunocytochemistry have also been applied to clinical diagnosis of neuromuscular disease. As enzyme histochemistry has become more widely available, formalin-fixed methods have become less frequently used in muscle biopsy work.
