Medical Image Databases covers the new technologies of biomedical imaging databases and their applications in clinical services, education, and research. Dialysis amyloidosis is a syndrome observed in dialysis patients characterized initially by joint pain, followed later by joint destruction. It is at present one of the few compliations in these patients. The factors involved in the genesis of this syndrome are not yet known with certainty and there is as yet no proven efficacious treatment otehr then renal transplantation. As a result, the interest in this topic among nephrologists remains very high. Dialysis amyloid gives general information on the pathophysiology, and the clinical aspects of all types of amyloidosis. It then goes on to discuss recently discovered B2, type of dialysis amyloidosis, from clinical and pathological features to physiological and pathophysiological aspects of this serious complication which effects patients suffering from chronic renal failure. In the past, many tumor marker laboratory tests have not been sensitive enough for the very early detection of cancer. However, many of them have nonetheless proved useful in monitoring therapy, following the course of the tumor, and predicting prog- nosis. Today, cancer may be viewed as a genetic disease with various specific chromo- somal and nucleotide aberrations, such as mutations, deletions, gene amplification, gene rearrangements, and translocations occurring during the transformation of a nor- mal cell into a malignant cell. The considerable advances in technology during the past several years have greatly enhanced our ability to detect human cancers in the very early stages of tumor forma- tion. These technologies include: (1) nucleotide molecular assays (genomics); (2) proteomics (multiplex protein measurements); (3) DNA microarrays; and (4) bio- informatics. Many of these technologies are already helping in the integration and use of multiple biomarkers for tumors. Although the individual biomarkers may reveal only limited information, the use of multiple biomarkers can help markedly elevate the diagnostic capabilities for early detection of tumors.
The accurate and reliable diagnosis of transmissible diseases is the most powerful weapon available to ensure their control, and in some cases eradication. The detection of parasites in clinical cases, companion and farm animals, and in the environment is relatively easy since many of them are visible to the naked eye, and those that are not are readily detected by light microscopy. Fungal infections can similarly be determined. Bacteria are somewhat harder to detect. Although their presence can frequently be detected by light microscopy, differential diagnosis, beyond their gross morphology, is almost always impossible. However, most bacterial pathogens can be cultured in the laboratory and can be accurately identified by combinations of a series of simple tests such as morphology, staining, antibiotic sensitivity, biochemical analyses, nutrient dependence, and phage sensitivity. Viruses, however, have proved much more difficult; their size and absolute dependence of the host cell for propagation have rendered useless the methods traditionally used for other microorganisms. Until the development of tissue culture in the middle of this century, diagnosis was entirely dependent on the skill and experience of the clinician. But this was an unreliable process since many of the common virus infections exhibit similar clinical symptoms, such as coryza, exanthema, vomiting, diarrhea, neuralgia, and lethargy. Indeed many viral infections display clinical signs that are indistinguishable from bacterial or parasitic infections.

Complex disorders of the carbohydrate metabolism and associated complications cause many abnormalities detectable by radiography in the bones and joints. Mild clinical symptoms associated with very severe radiological changes were first rec ognized in relation to the gastroenterologic complications of diabetes. This phenomenon is more frequent in the skeletal system. For example, mild and painless swelling of the foot joints may often mask extremely severe bone destruction. Several other bone changes associated with diabetes are only detectable by radiography. Thus, the radiologist plays an important role in confirming these diabetic complications, furthermore he is involved in the therapeutic management of the patient. Although many details on this subject have been published, however no summarizing monograph has yet appeared. Manuals discussing diabetes include only short reviews on complications of the osseous system. The fact that the incidence of diabetes is very high, at present 1 %-2 % of the population is affected and their number is gradually increasing - dis plays the timeliness of this subject. Fifty years of experience with insulin therapy indicates that several important problems still remain to be solved. Insulin and modern oral antidia betic drugs proved extremely efficient in the management of hyperglycemia and ketosis, but the incidence of other complications has not decreased. Moreover, as the number of diabetics and their life expectancy increase, late complications become likewise more fre quent. Diabetic osteoarthropathy is one of these complications.
