Management of Fractures in Severely Osteoporotic Bone - Orthopedic and Pharmacologic Strategies (Paperback, Softcover reprint... Management of Fractures in Severely Osteoporotic Bone - Orthopedic and Pharmacologic Strategies (Paperback, Softcover reprint of hardcover 1st ed. 2000)
Karl Obrant
R5,801 Discovery Miles 58 010 Ships in 10 - 15 working days

Over the past decade, major advances have taken place in the management of osteoporosis. Newdasses of pharmaceutical agents have been developed, tested, and used to treat millions of patients; education and early detection programs have been instituted around the world; and bone densitometry has received widespread recognition by healthcare agencies as being central to the preven tive health strategy necessary to care for an aging population. With all these advances, scientists continue to explore new methods to understand and treat this disease. The field of orthopaedic surgery has a great opportunity to bring this new information to its dinics and operating rooms. It is critically impor tant that the practicing surgeon be armed with this knowledge. Fractures are dearly the most common orthopaedic problem associated with osteoporosis. The goals of treatment are rapid restoration of mobilityand func tion and return of patients to a level of activity that supports their general health. The ability to control the metabolic condition or to treat the under lying cause of the osteoporosis with pharmacological intervention willimprove surgical results. Moreover, the special techniques required to succeed in the operative treatment of bone, which is already weakened by osteoporosis, must be carefully developed, tested, and understood. This book provides the first attempt to bring this body of information to the orthopaedic surgeon in a dearly organized and comprehensive way. Historically, few orthopaedic surgery or trauma training programs have offered much in the way of education in the science of bone metabolism."

This book is for postgraduate fellows, internists, and students interested in rheu- tology. It contains real cases and is designed to stimulate thought and further reading in this rapidly evolving specialty. I have included a series of common and uncommon cases that I have seen over the past decade in a busy consultative practice and have inserted up-to-date ref- ences and questions you may be asked on ward rounds or in clinic. My hope is that it will be a useful adjunct for physicians preparing for examinations or entering the field and will stimulate further interest. I would like to acknowledge my mentors Pierre Bouloux, MD, Tom Cooney, MD and thank Atul Deodhar, MD for his careful review of the manuscript. This book is dedicated to my parents and wife Batool who have made many sacrifices for which I remain eternally grateful. R1,324 R1,163 Discovery Miles 11 630 Save R161 (12%) Ships in 12 - 17 working days

Rheumatoid arthritis is a painful, incurable connective-tissue disease that attacks the hands and feet as well as the joints and may lead to deformities and permanent disabilities. Rheumatoid Arthritis: Plan to Win offers an inspiring, scientifically based game plan for minimizing the effects of this chronic illness, and ultimately, achieving optimal health. Here is the definitive resource for practical strategies and emotional support, whether you need help controlling pain, are concerned about medication side-effects, or want to locate a reliable RA news source or support group on the Internet. Helpful chapters detail the latest therapies, special dietary and exercise needs of people with RA, how to deal with arthritis at home and in the workplace, prepare for surgery, pregnancy and childbirth, enjoy healthy sexuality, use the mind-body connection to control symptoms, make informed decisions about alternative medicine, and perhaps most important, how to build a healthcare team and maintain excellent communication and working relationships with that team. By using this book as a starting point and a 24/7 reference guide, people with RA will be better equipped to form an effective plan of action, making well-informed decisions about their health along the way, and greatly enhancing their ability to live happy, productive lives. Cheryl Koehn, who was an Olympic-calibre athlete when she developed RA, is a leading advocate and spokesperson for people with arthritis. Together with her co-authors, John Esdaile, MD, and science writer Taysha Palmer, Koehn provides a wealth of information and practical advice, assembled from thousands of research papers as well as from her personal experiences with this debilitating disease.

Postgraduate Year One - Lessons in Caring (Paperback): Kenneth M. Heilman Postgraduate Year One - Lessons in Caring (Paperback)
Kenneth M. Heilman
R988 Discovery Miles 9 880 Ships in 12 - 17 working days

A world-renowned clinician, teacher, and researcher in cognitive and behavioral neurology, Dr. Ken Heilman has found that stories of his own initiation into the world of doctoring are one of the best ways to engage students and trainees about the common professional, medical, and ethical challenges they will face in daily practice. The twenty-five stories gathered here span the author's first year of clinical training at Bellevue Hospital during the 1960s following his graduation from the University of Virginia School of Medicine. That year- known in the past as internship or first-year residency and now called post graduate year one- is recognized as one of the most intellectually, emotionally, and physically demanding in a physician's life. For the author, it was to hold the most valuable lessons on caring for patients and to exert the greatest lasting influence on how he practices medicine. Each story in this book conveys a core lesson about the practice of medicine and also tells a wonderful tale- about how the author contracted tuberculosis because of a colleague's carelessness, a tough nurse who was a great teacher, a cardiologist who missed a diagnosis because of his arrogance, an acid-dropping ascetic who turned tricks on the side, a fellow trainee caught in a lie, and, as timeless a story as there ever was, the utter impossibility of finding a parking spot in New York City, among others.

Your complete guide to understanding and living well with rheumatoid arthritis

If you or someone close to you suffers from rheumatoid arthritis (RA), this book is a powerful tool you don’t want to be without. Two of the authors are physicians at the Hospital for Special Surgery, one of the nation’s oldest and best orthopedic research hospitals. Its purpose is to arm you with the information you need to take charge of your disease management and work with your doctors to develop a treatment program that’s right for your lifestyle and life goals.

In plain English, the authors demystify rheumatoid arthritis–its causes, progression, and various effects on the human body–and decode the often perplexing language that doctors use to describe RA and its diagnosis and treatment. They explain the various treatment and pain management options available to you and ways to discuss them with your doctor. The authors also include the true stories of RA patients whose tales offer encouragement and powerful insights for dealing with the difficult psychological and lifestyle issues RA sufferers face. Their expert advice and guidance covers a range of topics of vital concern to people affected by RA, including:

  • Old, new, and emerging drug therapies, including gold, corticosteroids, and immunosuppressants
  • Analgesics and anti-inflammatories
  • Alternative treatment strategies
  • Nutrition, diet, and exercise for RA
  • Vitamins and supplements, including glucosamine and Evening Primrose oil
  • Managing crucial lifestyle issues such as sports, travel, and sex
  • Pregnancy and RA
  • Alleviating depression and pain
  • Surgical options, including joint replacement therapy

Section A: Methods of Autoantibody Detection This Section provides well structured protocols for autoantibody detection. Step-by-step procedures are accompanied by explanatory notes and comments, clear diagrams, line illustrations and excellent photo illustrations. Extensive literature references lead the way to further background information. The methods presented were validated by more than 40 leading laboratories active in sera analysis, which indicates that these methods have been found to be practically useful and lead to consistent inter-laboratory results: consensus in autoantibody detection. Section B: Autoantigens This Section contains the first compilation of full, detailed information on autoantigens related to important autoimmune diseases. The chapters are all structured according to an easy-reference fixed template structure: specific detection methods, cellular localization, biochemical characteristics, function, cDNA and derived amino acid sequence, gene structure, B- and T-cell epitopes and lists of published monoclonal antibodies. The text is greatly enhanced by many beautiful schematic figures and photo illustrations. As in Section A, extensive literature references are provided. The information for this section was brought together by leading experts in their fields. This Manual is an ideal text and reference book for bench scientists working in the field of autoimmunity, but also for rheumatologists, general (internal medicine) physicians or clinical immunologists caring for patients with autoimmune disorders.

The Rheumatoid Hip (Paperback, Softcover reprint of the original 1st ed. 1990): Heiner Thabe The Rheumatoid Hip (Paperback, Softcover reprint of the original 1st ed. 1990)
Heiner Thabe; Translated by Julia Roseveare, David Roseveare
R2,929 Discovery Miles 29 290 Ships in 10 - 15 working days

Rheumatoid arthritis demands intensive interdisciplinary cooperation between the orthopaedic surgeon and internist, and this monograph provides all the information necessary for the specialist dealing with inflammatory changes of the hip joint in rheumatoid arthritis. The reaction of the joint to rheumatoid arthritis is generally uniform, but the diagnoses may vary. For this reason, particular attention is paid to the differential diagnosis of the chronic inflammatory changes at the hip joint. The course of the disease is outlined, so that varying treatment approaches can be understood. The book describes the medical and the physical treatment of the hip joint, including discussion of the mechanisms of action and side effects that may occur. The options for conservative orthopedic treatment and for operative intervention are carefully considered. The broad spectrum of treatment principles, courses of action and results that are discussed will aid every physician interested in the rheumatoid hip.

The so-called 'connective tissue disorders' hold a particular fascination to both the rheumatologist and the dermatologist, and the diagnosis of, sometimes subtle, changes in the skin can often be difficult. This book aims to give a succinctaccount ofcutaneous manifestations of the major connective tissue diseases, including the cutaneous side effects of anti-rheumatic therapy, and the main diagnostic and prognostic features and guides to their management. Although the book is intended primarily for dermatologists and rheumatologists, it will also be of value to general physicians and general practitioners.

The main research described in this book is concerned with assessing the interrelationship of psychological adjustment, compliance with medical advice and use of unorthodox treatments in a sample of rheumatoid arthritic out-patients. Basically this research is comprised of four components: two pilot studies; the development and postal administration of an illness knowledge assessment questionnaire; and a 'follow-up' correlational study. pilot study 1, which involved interviews with 18 patients, was a 'question seeking' study - the aim being to develop research questions of relevance to the Subject sample. The findings of this study suggested that poor illness knowledge and consequent misconceptions was contributing to psychological distress and affecting health behaviour among patients. In pilot study 2 a sample of 17 patients were questioned in detail - but using open-ended questions - about their understanding of their condition and treatment. The results of this study supported the impression gained in pilot study 1, that patients were typically ill-informed about their illness and tended, in the absence of correct information, to overestimate "the risks associated with rheumatoid arthritis and underestimate the value of treatments.

This selection of MCQs in rheumatology is largely aimed at MRCP candidates but some will be difficult for established rheumatologists and some will be possible for enthusiastic medical students. The format of the questions largely follows that used in the MRCP Part I examination with a few exceptions employing diagrams and assertion/reason questions (these are explained on p. vi). No gui dance on scoring is included since we feel that if an honest attempt is made at answering the question then the process is, in itself. educa tional. Some of the answers have been determined only after consider able discussion between us and as far as possible we have attempted to convey the views expressed in accepted British texts. Where answers seem contentious an appropriate reference has been given. Similarly the layout of the chapters largely follows that adopted by standard text books although the basic sciences have been incorpor ated within the individual chapters. This volume reviews the state-of-the-art in osteoarthritis. The pathogenetic roles of mechanical, systemic and inflammatory factors and the importance of neurogenic factors in pain management and in the causes of disease are considered. Of particular value is the review of the implications of pathogenetic mechanisms in future modes of management. The concept of "activated arthrosis" is given special attention. Finally, controversial issues and new areas for investigation are discussed. The book will help the clinician and the investigator consider the importance of addressing different phases and subtypes of osteoarthritis in planning treatment.
