A partire dagli anni '80, numerosi ricercatori nel campo della riabilitazione dei disturbi del linguaggio del cerebroleso adulto hanno introdotto la distinzione tra competenza linguistica e competenza comunicativa. Gli stessi autori hanno ipotizzato che l'approccio riabilitativo, ivi inclusa la valutazione del paziente, non dovrebbe essere indirizzato esclusivamente al recupero di abilita linguistiche ma contemplare anche trattamenti volti a sviluppare le capacita di interazione comunicativa in particolari contesti. Il disturbo afasico danneggia le capacita comunicative (e spesso anche lavorative) e non e migliorato, tranne eccezioni, dalle terapie mediche disponibili. Le possibilita di incidere su questo deficit sono dunque affidate, per il momento, alla logopedia.
